Job summary

Woodsley and Holt Park PCN cover a large diverse population of approximately 110 000 patients in Leeds. Although we are two PCNs we essentially work as one with an over-arching leadership team which is accountable to both PCNs. Our PCN combined comprises of ten GP practices.

We are a dynamic, motivated and enthusiastic PCN who have successfully introduced large teams of social prescribers, physiotherapists, pharmacists and pharmacy technicians delivering high quality healthcare across our PCN. We have also just recruited occupational therapists, podiatrists, dieticians as well as health & wellbeing coaches. This has been through a combination of senior clinical leadership and local GP practice integration.

We are looking to recruit a care co-ordinator to work in our new and developing Ageing Well Team. At present the team comprises of occupational therapists, community matrons, physio, and clinical care coordinator. This provides an exciting opportunity to provide rewarding integrated care to this vulnerable cohort of patients. You will work dynamically with our GP practices to contribute to an innovative way of working, meeting the healthcare needs of our frail population and enable people to remain as independent as possible in their owns homes.

Main duties of the job

The post holder is expected to take a lead on providing administrative support to the team. This will include proactively identifying patients through clinical searches, developing systems and processes to support clinical practice and liaison with patients, carers and other professionals.

Main Duties:

To help to develop the booking system and take responsibility for booking patient appointments.

To support clinicians to make referrals and signpost to specialist services quickly where necessary.

To monitor and record outcomes of all those accessing the service.

To gather and collate statistical and other information and data as required, to report on activity and outcomes and ensure effective qualitative and quantitative monitoring and evaluation of the service.

To be involved with conducting patient searches to identify population groups who would benefit from our service

To support clinical staff in the identification and completion of QOF targets that can be carried out as part of the holistic assessments.

Identify and set up productive partnerships, and formalise partnerships with local people, community groups and other agencies that may have an impact on health and wellbeing.

Take part in internal and MDT meetings as required.

To work collaboratively with GP practices and other PCN staff as required

To be involved with collation of service user and carer feedback.

To support clinical visits where a second person is required

There will be the opportunity to develop your role as the team becomes more established. This may include introducing clinical elements such as clinical observations training can be provided and home visits to support health and social care needs.

Communication

To provide, and receive, information with empathy and understanding, and to adapt communication when barriers to understanding arise seeking senior guidance when necessary.

Take an active role in MDT processes providing administrative support and input as required.

Provide a point of contact for the person and their family carers and the range of professionals involved in their care.

To develop and maintain effective working relationships with other members of the primary care and wider team.

Administration

Keep timely, accurate and complete electronic patient records and activity data, all of which will be maintained and stored in accordance with professional and organisational standards.

To have responsibility for appropriate documentation related to patients in their care to be disseminated to all relevant professionals in a timely manner and in accordance with operating standards.

Other

To participate in discussions about the direction of service developments and improvements.

To participate in service audits and changes.

To participate in discussion for the implementation of policies and procedures

To contribute to the health and safety of others and the environment

To participate in appraisal processes and participate in CPD/ personal development plan.

To pro-actively participate in mandatory training and in-service training.

Responsibilities and Confidentiality

The post holder is expected to:

Adhere to Practice PCN policies and procedures and relevant legislation.

Attend mandatory training as identified by the Practice PCN

The post holder must maintain the confidentiality of information about patients, staff and Practice business in accordance with the General Data Protection Regulation, Caldicott principles and Practice Information Governance policies and procedures.

Safeguarding

All employees have a duty for safeguarding and promoting the welfare of children and adults at risk. Staff must be familiar with the practice PCNs Safeguarding Policy and the process for raising concerns about the welfare of anyone with whom they have contact. Staff must also ensure they receive the appropriate level of safeguarding children and adult training.

Staff are expected to access supervision and support from their line managers and or the locality based safeguarding team when managing complex cases.
