This role will be hosted by South East Leeds GP Group, and be based in up to 2 GP surgeries in either Beeston Primary Care Network (PCN) or Middleton & Hunslet Primary Network (PCN) which covers the following 8 GP Practices located in LS11 and LS10:

- City View Medical Practice

- Beeston Village Surgery

- Oakley Terrace Surgery

- Lingwell Croft Surgery

- South Bank Surgery

- Arthington Medical Practice

- Church Street Surgery

Main duties of the job

The Nurse Associate will work in accordance with the NMC code of conduct and be responsible for the delivery of basic practice nursing services both independently and in participation with the Practice Clinical Team within the PCNs practices.

The Nurse Associate will work with support from the Practices and PCN Team to deliver, organise and facilitate quality care appropriate to individual patient needs.

The Nurse Associate will work under national, local and practice policies to ensure patients health needs are met and support the PCN Frailty Team to provide personalised & anticipatory care plans for patients with Severe Frailty, in accordance with the NHS Long Term Plan

As part of this role, you will be expected to work across various sites and settings within the community e.g., you could work in care homes, within a practice, or from a community venue within the PCN. You may on some occasions be required to work evenings and weekends to meet the needs of the patients.

JOB SUMMARY

The Nurse Associate will work in accordance with the NMC code of conduct and be responsible for the delivery of basic practice nursing services both independently and in participation with the Practice Clinical Team within the PCNs practices.

The Nurse Associate will work with support from the Practices and PCN Team to deliver, organise and facilitate quality care appropriate to individual patient needs.

The Nurse Associate will work under national, local and practice policies to ensure patients health needs are met and support the PCN Frailty Team to provide personalised & anticipatory care plans for patients with Severe Frailty, in accordance with the NHS Long Term Plan

As part of this role, you will be expected to work across various sites and settings within the community e.g., you could work in care homes, within a practice, or from a community venue within the PCN. You may on some occasions be required to work evenings and weekends to meet the needs of the patients.

Key ResponsibilitiesClinical Appropriate Training will be provided The Nurse Associate will contribute to the ongoing assessment of care, promote health and well- being of patients attending the practice and able to recognise when to refer to a Senior Colleague

Assist in the screening for patients at risk of long-term conditions.

Participate and undertake routine and opportunistic health screening assessments, providing health advice and support making appropriate referrals into exercise, alcohol, smoking and weight management

Provide Health Promotion for patients with Long Term Conditions. Make appropriate referrals as necessary in accordance with practice and national guidelines.

Work with patients to support compliance with and adherence to prescribed treatments.

Cervical Cytology following full training

Undertake Mental Health and Learning Disability Annual Health Checks.

Undertake ECGs as per practice policies or referrals from the senior nurse or GP.

Perform spirometry testing for diagnosis as per practice and national guidelines following full training.

Undertake training for all vaccination and immunisation programmes for both adult and children

Autonomously, administer medications or injections via a prescription or patient specific direction.

Assist in the delivery of the seasonal flu or national vaccinations campaigns as required by the Practices via a PSD

Following Guidance from the Practice Nurses implement treatment for patients presenting with acute wounds and chronic leg Ulcer Management.

Undertake doppler assessments

Undertake the collection, storage and despatch of pathological specimens to high standard ensuring universal precautions are followed and manage the results of these samples, referring to GP as necessary. Perform any investigatory procedures requested by the GPs or opportunistically if clinically indicated from patients records.

Participate in support and supervision of students. Referring any concerns or development needs to the Student Supervisor or Practice Manager

To carry out Home Visits as required

Training & Development

The Nurse Associate must to assess with Line Manager their own learning needs and undertake learning as appropriate

The Nurse Associate must complete all mandatory training within agreed time scale.

To attend training and development courses according to practice requirements and availability of mentorship support and funding available.

To support Student Nurses.

To participate in training at in-house TARGET sessions with the nurse team.

To keep up to date with local and national developments.

To participate in the development of practice policies and guidelines.

Governance

  • Produce complete and accurate records of patient consultation, in line with best practice, confidentiality, policies and procedures
  • Deliver care according to NHS guidance, NICE guidelines and evidence-based care
  • Take part in the maintenance of quality governance systems and processes across the PCN and its activities.
  • Utilise the audit cycle as a means of evaluating the quality of the work of self and the team, implementing improvements where required.
  • Work with other clinical teams on improving the quality of healthcare in response to local and national policies and initiatives as appropriate
  • Evaluate patients response to health care provision and the effectiveness of care
  • Support and participate in shared learning across the PCN and wider organisation
  • Manage, review and identify learning from patient complaints, clinical incidents and near-miss events
  • Awareness of statutory safeguarding, notification processes and local guidance for children/vulnerable patients, applying relevant policies and legislation to protect them
