Job responsibilities

Middleton & Hunslet PCN is seeking an Advanced Clinical Practitioner (ACP) to be part of our multi-disciplinary and multi-agency team of professionals. We are looking for an individual who is passionate about Primary Care and is keen to work across traditional health and social care boundaries. This is open to anyone with relevant registration. This is an exciting and unique opportunity to be at the forefront of the service delivery of our new PCN Team, including the Frailty Hub. Working both independently, and within our multi agency team, the role will operate across the traditional health and social care organisational boundaries, including our GP practice partners, to deliver a holistic approach to acute on day / rapid response services, enhanced care into care homes and enhanced proactive care for older people with frailty. We are looking for an Advanced Clinical Practitioner with a strong interest in working in Primary Care and who aspires to provide a continuous improvement approach to delivering responsive, safe, patient-centred, and effective care within a team. The service, within which the post holder would work, supports patients with multifaceted health problems living in their own homes as well as in care homes. The service liaises with care home staff, carers, families, and colleagues to maintain and maximise individualised patient-centred care (including advanced care planning). It also supports General Practice in delivering exciting new developments and this can be flexible depending on skills, expertise, and interests.

Requirement

• Autonomous practitioner Undertake comprehensive health assessments, initial history taking, clinical assessment, physical examination, diagnosis, treatment and evaluation of their care
• Use advanced clinical decision making and assessment skills, coordinate care planning and initiate onward referrals as appropriate
• Demonstrate sound safe clinical decisionmaking.
• Prescribing practitioner.
• Able to consult with all age ranges.

Main duties of the job

• An ability to make professional autonomous decisions for which they are accountable.
• An ability to assess, diagnose and treat patients with undifferentiated and undiagnosed problems.
• Screen patients for disease risk factors and early signs of illness.
• Develop, with the patient, an ongoing care plan with emphasis on health education and preventative measures.
• Order and follow up on necessary investigations providing treatment and care both individually, as part of a primary care team and through referral to other agencies
• Work collaboratively with other health care professionals and disciplines
• An ability to provide clear and accurate documentation
• An appreciation of the importance of confidentiality
• The ability to assess for safeguarding issues and proactively refer on to relevant organisations / safeguarding teams.
• An acknowledgement of the importance of equality, diversity and the rights of patients, carers and colleagues

Enhanced Frailty

• Utilise population health management (PHM) approach to review severely frail people in their own homes and complete annual clinical reviews.
• Develop andimplement an MDT/multi-skilled approach to support the PHM severe frailty cohort, working with other healthcare professionals including the Neighbourhood Teams, Geriatricians, Mental Health Teams, care coordinators and PCN Clinical Pharmacists.

Enhanced Health in Care Homes

• Work with the PCN Care Home teams to support the MDT meetings and completepersonalised care plans for residents and review them clinically as necessary.

Teamwork and mutual support are integral to our working practices. Our PCN works on many joint projects with our neighbouring Beeston PCN, which is made of 46,000 patients across 4 GP Practices in Beeston.

Job description and responsibilities

Quality Assurance & Risk Management

Working closely with the PCN Leadership Team, GP Partners and Practice Managers, you will support the delivery of the following: -

• Ensure all care and services are always delivered to the highest standards and across the CQC domains, Safe, Effective, Caring, Well-led and Responsive
• Produce accurate, contemporaneous, and complete records of patient consultation, consistent with legislation, policies, and procedures
• Prioritise, organise, and manage own workload in a manner that maintains and promotes quality and service expectations delivering health service
• Assess and triage patients, including same day triage, and as appropriate provide definitive treatment (including prescribing medications following policy, patient group directives, NICE (national) and local clinical guidelines and local care pathways) or make necessary referrals to other members of the primary care team
• Manage undifferentiated undiagnosed condition and identify red flags and underlying serious pathology and take appropriate action
• Use complex decision making to inform the diagnosis, investigation, complete management of episodes of care within a broad scope of practice
• Deliver care according to NICE guidelines and evidence-based care
• Evaluate patients response to health care provision and the effectiveness of care
• Support and participate in shared learning across the practice
• Assess the impact of policy implementation on care delivery
• Understand and apply legal issues that support the identification of vulnerable and abused children and adults, and be aware of statutory child/vulnerable patients health procedures and local guidance

Management of risk

• Manage and assess risk within the areas of responsibility, ensuring adequate measures are in place to protect staff and patients
• Monitor work areas and practices to ensure they are safe and free from hazards and conform to health, safety and security legislation, policies, procedures and guidelines
• Apply infection-control measures within the practice according to local and national guidelines
• Interpret national strategies and policies into local implementation strategies that are aligned to the values and culture of general practice

Communication

• Utilise and demonstrate sensitive communication styles, to ensure patients are fully informed and consent to treatment
• Communicate with and support patients who are receiving complex or emotionally sensitive information
• Communicate effectively with patients and carers, recognising the need for alternative methods of communication to overcome different levels of understanding, cultural background and preferred ways of communicating
• Anticipate barriers to communication and take action to improve communication
• Maintain effective communication within the practice environment and with external stakeholders
• Ensure awareness of sources of support and guidance and provide information in an acceptable format to all patients, recognising any difficulties and referring where appropriate

Qualifications

Essential

• - RGN/Paramedic Qualification
• - NMC 1st level Registration
• - Educated to Masters Level Nurse Paramedic Practitioner Pathway
• - Independent Prescribing Qualification
• - Evidence of continued professional development

Desirable

• - Accredited training in chronic disease management
