The post holder will be employed by South East Leeds GP Group, but will be deployed and managed day to day from within the Middleton & Hunslet Primary Care Network

Working both independently, and within our multi agency team, the role will operate across the traditional health and social care organisational boundaries, including our GP practice partners, to deliver a holistic approach to acute on day / rapid response services, enhanced care into care homes and enhanced proactive care for older people with frailty.We are looking for an Advanced Clinical Practitioner with a strong interest in working in Primary Care and who aspires to provide a continuous improvement approach to delivering responsive, safe, patient-centred, and effective care within a team.

Our 2 Primary Care Networks provide new and additional health and wellbeing services to our patients. Working in partnership with our 8 GP Practices, we deliver extra services such as wound care, physiotherapy, mental health support, wellbeing clinics, respiratory reviews, BP monitoring, frailty home visits, medication reviewsand other innovations.
Beeston PCN and Middleton & Hunslet PCN were established in 2019, and weve been working together ever since.

It also supports General Practice in delivering exciting new developments and this can be flexible depending on skills, expertise, and interests.
Requirement
- Autonomous practitioner Undertake comprehensive health assessments, initial history taking, clinical assessment, physical examination, diagnosis, treatment and evaluation of their care
- Use advanced clinical decision making and assessment skills, coordinate care planning and initiate onward referrals as appropriate
- Demonstrate sound safe clinical decisionmaking.

- Utilise population health management (PHM) approach to review severely frail people in their own homes and complete annual clinical reviews.
- Develop andimplement an MDT/multi-skilled approach to support the PHM severe frailty cohort, working with other healthcare professionals including the Neighbourhood Teams, Geriatricians, Mental Health Teams, care coordinators and PCN Clinical Pharmacists.
Enhanced Health in Care Homes
- Work with the PCN Care Home teams to support the MDT meetings and completepersonalised care plans for residents and review them clinically as necessary.

Working closely with the PCN Leadership Team, GP Partners and Practice Managers, you will support the delivery of the following: -
