Job responsibilities
JOB PURPOSE
The PCN Care Coordinator Phlebotomist hybrid role is seen as
a critical and evolving post to support the Primary Care Network member GP
practices and PCN Multi-Disciplinary Teams (MDTs) to deliver effective,
co-ordinated care for patients. This will involve a mix of working from GP
Practices, community settings and home visits.
Key working relationships

Core duties and responsibilities:
Working
strictly in accordance with specific guidelines and agreed protocols, the Care
Coordinator Phlebotomist will assist with the ongoing support of patients, families,
and carers in a professional manner - using resources economically and
efficiently.
Participate
in multidisciplinary and case management meetings, where appropriate, and
develop and maintain effective working relationships with health and social
care colleagues and other agencies to ensure that patients and those with long
term conditions receive a consistent, integrated response to all
contacts/referrals.
Phlebotomy
Ensuring that the
patient is as comfortable and relaxed as possible and confirming their
agreement to the collection of a blood sample
Collection of samples
of blood in accordance with training and procedures
Ensuring samples are
stored in appropriate containers
Accurate, legible
labelling of all samples to confirm patient identity
Checking patient
identification against the request and confirming with patient whenever
possible
Recording collection
of blood sample in patient record in accordance with practice procedure
Ensuring that samples
are dispatched to the appropriate laboratories in a timely fashion
Ensuring that
anomalies are reported
Notifying the
clinician of any significant patient issues
Liaising with and
assisting members of the administrative team

any location in the community in order to meets service needs

Information, which provides guidance on the use and disclosure of information. South
& East Leeds General Practice Group also has a range of policies for the
use of computer equipment and computer-generated information. These policies
detail the employees legal obligations and include references to current legislation.

Risk management
You will be responsible for adopting the risk
management culture and ensuring that you identify and assess all risks to your
systems, processes and environment and report such risks for inclusion within
the South & East Leeds General Practice Group risk register. You will also
attend mandatory and statutory training, report all incidents/accidents,
including near misses, and report unsafe occurrences as laid down within South
& East Leeds General Practice Group Incidents and Accidents Policy.
No smoking policy
South & East Leeds General Practice Group is
a no smoking organisation therefore staff are not permitted to smoke whilst
on duty.
All staff are expected to recognise their role
as ambassadors for a healthy lifestyle. As such, staff should not smoke on South
& East Leeds General Practice Group or other healthcare premises.
Other duties
There may be a requirement to undertake other
duties as may reasonably be required to support The Middleton & Hunslet
Primary Care Networks additional health and wellbeing care service objectives.
