Job summary
LS25/26 Primary Care Network (PCN) is one of two PCNs in Leeds
which is working with Dementia UK to employ and integrate the Admiral Nurse
role into Primary Care. This is a model which has already been trialled
successfully in other parts of the country.
Main duties of the job
The role will provide a high level of care and support for people
living with dementia, and their families, to maintain and enhance people’s
quality of life. It will also build on the skills and knowledge of primary care
and care home staff in our area.
Admiral Nurses are
specialist dementia nurses who offer expert practical, clinical and emotional
support to both people with dementia and their families / informal carers.
For the Admiral
Nurse Clinical Lead post we are looking for an exceptionally skilled Registered
Nurse (adult/mental health/learning disability) with a passion for, and
significant demonstrable track record of, providing person-centred and
relationship-centred dementia care with a strong sense of advocacy and
equalities for this client group. Combining direct work with families with
supporting the best practice of health and social care professionals, you will
call upon your impressive knowledge of bio-psychosocial interventions to
improve the experience and quality of life of people and families affected by
dementia.
Dementia UK will
support you to maintain and strengthen your clinical and leadership expertise
as a specialist Admiral Nurse through regular clinical supervision and practice
and professional development opportunities.
About us
SEL GP Group will employ the Admiral Nurse who will be deployed to LS25/26 PCN, working across the PCN’s 7 practices and 12 older people’s care homes. There will be close links with the community neighbourhood teams, memory support workers, the older person’s mental health team and voluntary sector organisations.
The Admiral Nurse in LS25/26 will be part of our growing team of professionals based within the primary care network working together with GP practices to deliver holistic care. The Admiral Nurse will be part of our multi-disciplinary team and work alongside nurses, therapists, primary care, social care and third sector providers, amongst others, you will also benefit from being a part of a Clinical Network of Admiral Nurses working across West Yorkshire and beyond.
The post holder
will need to have the ability to work in a variety of settings, across our PCN.
The role will include seeing individuals, carers and families individually, and
providing education/ training, as well as balancing a clinical caseload - all
whilst setting-up, developing, maintaining and evaluating a new service.
