Job summary
An exciting opportunity has arisen for a Nurse Associate to
support Crossgates Primary Care Network (PCN).
You will be a registered Nursing Associate with the NMC and will
have completed your foundation degree. The PCN Nurse Associate will work
closely with a multidisciplinary team to support the delivery of key priorities
around prevention, tackling health inequalities and providing high quality patient
centred care in line with the National Enhanced Direct Service requirements.
Main duties of the job
In line with best practice and with support from the MDT, you will
carry out various procedures to monitor and improve the health of patients.
Including: physical observation, venepuncture, ECG recording, injections,
annual reviews and other procedures in line with your scope of practice.
You will work in collaboration with a range of other
professionals, including GPs, practice nurses, pharmacists, paramedics,
community nurses and other PCN services.

About us
Crossgates PCN includes
four practices working over seven sites: Ashfield and The Grange Medical
Centre, Colton Mill and The Grange Medical Centre, Family Doctors and Manston
Surgery, and you will be expected to provide clinical input to patients registered
across all the practices in the PCN. The PCN has a patient population of 30,000
including patients living across 5 care homes.

- Ability to perform basic physiological observations including BP, pulse checks, sats, respiration and peak flow readings.
