We have a new and exciting opportunity for an Admiral Nurse to join Cross Gates PCN to support patients living with Dementia and Severe Frailty. Admiral Nurses are specialist dementia nurses who offer expert practical, clinical and emotional support to both people with dementia and their families / informal carers. You will be working closely with our local care partnership to deliver this.
Main duties of the job
For the Admiral
Nurse post we are looking for an exceptionally skilled Registered Nurse
(adult/mental health/learning disability) with a passion for, and significant
demonstrable track record of, providing person-centred and relationship-centred
dementia care with a strong sense of advocacy and equalities for this client
group. Combining direct work with families with supporting the best practice of
health and social care professionals, you will call upon your impressive
knowledge of bio-psychosocial interventions to improve the experience and
quality of life of people and families affected by dementia.
The post holder
will need to have the ability to work in a variety of settings, across Cross
Gates Primary Care Network. The role will include seeing individuals, carers
and families individually, running groups and providing education/ training, as
well as balancing a proxy-clinical caseload - all whilst setting-up,
developing, maintaining and evaluating a new service.
Dementia UK will
support you to maintain and strengthen your clinical and leadership expertise
as a specialist Admiral Nurse through regular clinical supervision and practice
and professional development opportunities. You’ll become a welcome part of a
network of Admiral Nurses and will have the opportunity to access relevant
post-graduate training and develop professionally, informed by the Admiral
Nurse Competency Framework.
About us
As a PCN Admiral
Nurse with specialist and enhanced knowledge around Dementia, you will lead on
Dementia Care for a patient population of 32,000 and support the wider primary
care team across 4 GP practices - Colton Mill and The Grange Medical Centre,
Ashfield and The Grange Medical Centre, Family Doctors and Manston Surgery.
Our PCN Executive
Leadership Team will support you to understand our population needs and key
priority areas. You will be part of a wider PCN team which includes a Clinical
Director, GP Lead for Frailty and care homes, Lead Nurse, PCN Manager, Advanced
Clinical Practitioner, Nurse Associates, Pharmacists, Pharmacy Technicians,
Paramedic, Mental Health Practitioner and Care Coordinators. We have 5 care
homes aligned to our PCN and have developed close partnership working to
deliver an Enhanced Care Home Service.
