Job summary
We are looking for further clinical pharmacists to join the existing PCN teams in East Leeds Collaborative: Seacroft - 1 post, Crossgates - 2 posts and York Rd - 1 post.
The ELC team consists currently of 2 senior and 1 clinical pharmacist and 3 pharmacy technicians, and is supported by 3 Clinical Directors. There are also 3 practice based pharmacists and 1 practice based technician.
These roles are new and exciting, with a real opportunity to develop and use your clinical pharmacy skills in a patient-facing environment. You will have the opportunity to support new initiatives and projects across East Leeds Collaborative as they develop. SEL GP Group is the ideal employer for these roles as you will benefit from peer support from an extended team of 17 pharmacists and 7 technicians, and mentoring from the Head of Clinical Pharmacy.

Main duties of the job
The pharmacy team aims to provide the best patient care possible. You will work in a patient-facing role, supporting patients to manage long-term conditions effectively through SMR. You will work as part of the MDT and provide support to practices, care homes and patients with medication queries, medicines policies and procedures, QOF/local incentive schemes, prescribing processes, audit and medicines safety.
You will be a great team player, but also capable of independent, autonomous work. You will have excellent inter-personal skills. You will have excellent communication skills and be committed to providing exceptional patient-care. Experience of general practice is desirable but not essential.
About us
South and East Leeds GP Group employs these posts on behalf of our seven member PCNs. We employ an established team of practice-based and PCN-based clinical pharmacists and technicians and are ideally placed to support you in this new role.
You will be supported in your post with mentoring from the Head of Clinical Pharmacy for SEL GP Group, regular pharmacy peer support meetings involving all the PCN and practice pharmacists and technicians working in SEL GP Group and also the Leeds GP Pharmacy Network (80+ members). You will be allocated a GP clinical supervisor and will also have support from the Leeds GP Confederation pharmacy team.
We have very close relationships with specialist and consultant-level pharmacists working in Leeds and so are able to provide high level clinical teaching and support.
All PCN pharmacy staff are required to enrol or (or be exempted from) the CPPE Primary Care Education Pathway.

The post holder will work within their clinical competencies as part of a multi-disciplinary team to provide expertise in clinical medicines optimisation across the PCN. This will include structured medication reviews with direct patient contact and may include contributing to:
Management of long-term conditions
Management of medicines on transfer of care
Reviewing systems for safer prescribing
Contributing to repeat prescription authorisations and reauthorisation and actioning acute prescription requests
Addressing both the public health and social care needs of patients
Minor illness
Contributing to achievement of QOF and locally commissioned quality improvement schemes
Undertaking clinical audit
