Job summary

FCMS has created a home monitoring service for patients with long standing conditions e.g., COPD, Heart Failure, T2DM. This will be to remotely monitor patients who are in a Care Home or Housebound to support their GP practice.

Main duties of the job

Duties and Responsibilities

• To support qualified clinicians in supporting patients on the home monitoring services

• To provide remote advice for patients with long standing conditions with support of qualified clinicians

• Input into holistic care plans, in order to ensure a focus on the individual’s goals and support them to comply with the plan ultimately improving health outcomes

• Patient education and health promotion

• Work to attain the service KPIs and targets

• Ensure all patient contact is recorded in a timely manner on the IT systems to enable tracking of patient outcomes, both in real-time to support ongoing intervention planning and retrospectively to understand the impact of the service

• To ensure mandatory training is kept up to date

• Acknowledge and accommodate barriers to communication and understanding including speech, hearing, language and emotion.

• Flexibility in attending organised educational updates and eagerness to expand on current knowledge base and cascading new educational information to colleagues whenever possible

• Inform the Clinical Manager of any untoward incidents Work within all relevant policies and procedural guidelines e.g., infection control, safeguarding, chaperoning and risk management

“COVID-19 has illustrated starkly the devastating impacts on those vulnerable to viruses and infections, in a very real and public forum”.

It has also shown a health service capable of making huge leaps in innovative working methods to adapt quickly to changing dynamic needs. As a forward-thinking organisation, we are progressing new ways of working and consulting with patients.

FCMS has created a home monitoring service for patients with long standing conditions e.g., COPD, Heart Failure, T2DM. This will be to remotely monitor patients who are in a Care Home or Housebound to support their GP practice. Patients will initially have a face-to-face assessment in their home environment to create a care plan and ensure we have all the information needed for safe admission onto the service. Patients will have access to observation equipment and input their readings which will alert a clinician if any problems.

We are working collaboratively with Blackpool Teaching Hospital to provide home monitoring to patients:

- Awaiting / undergoing cardiac procedures

- Patients discharged with respiratory conditions needing further monitoring

Within the primary care development unit, we offer health checks on a mobile unit, this involves patient observations and ear syringing.

Job Summary

We are looking for forward thinking, motivated Health Care Assistants who have a passion for new ways of working whilst providing safe patient care. The Health Care Assistants will provide support qualified clinicians monitoring alerts and some face-to-face monitoring of patients in their home environment. Our aim is to work closely with the local trust, community matrons, heart failure team, GP Practices, Rapid Response, and other teams in the community to provide optimum patient care.

The role is subject to a DBS check which will require three forms of valid ID to be produced and verified. The onboarding process is also subject to an Occupational Health check, suitable professional references and eligibility to work in the UK (with the requirement to provide relevant documentation as evidence).

The organisation is committed to safeguarding and promoting the welfare of children, young people and vulnerable adults and expects all staff to share this commitment. You will be expected to fulfil your mandatory safeguarding training at the level applicable to this role.
