As a District Nursing Team, we cover around 40,000 patients across the Grimsby and Cleethorpes area for the Freshney Pelham PCN Practices. Each surgery is represented on our Board of Directors by a nominated GP.

We currently have 42 staff consisting of Nurses, Assistant Practitioners, HCA's and an Admin team. We have recently created a Long Term Condition Team and a Care Home Team and are now looking to grow our Nursing Team further.

As a provider of health care, our commitment is to deliver services to the population which include nursing care, support and advice, signposting, and preventative services to enable independence for as long as possible.

We are now looking to recruit a Nurse Associate to join our team; this is a fantastic opportunity for a newly qualified or experienced Nurse Associate with a full preceptorship programme and training is available. 

Main duties of the job

  • Contribute to the assessment, management and evaluation of the health and well-being of patients on a caseload as delegated by DN Operational Team Leader to support and improve the care of patients with long term conditions.
  • Work within care plans established for patients, and have the ability to recognise the changes in a patient’s condition / deterioration which require intervention and understand when to refer for reassessment of patient care within scope of clinical competency.
  • Support the assessment and management of patient needs through data collection, monitoring of patients progress, discussion and documentation. Participate in care reviews with the clinical team as required.
  • Ensure high standards of patient care are given and maintained and be involved in initiatives to improve patient care.
  • Establish and maintain effective communication with individuals and groups.
  • Discuss with the patient any issues relevant to their health and social care needs.
  • Demonstrate a good understanding of the principles of consent & ensure valid consent is obtained prior to undertaking nursing & care procedures.
  • Act as an advocate for the patient and family, including promoting and protecting their interests, privacy and rights, equality and inclusion.
  • Demonstrate proficiency to administer medications. 

About us

We are a growing community nursing service which has 6 teams. Four teams are aligned directly to our PCN practices and we also have a specialist Long Term Condition Team and a Care Home Team all of which are led by a District Nurse Operational Team Leader and managed by the Clinical Lead Manager, an Operational Business Manager and our District Nurse Practice Educator.

We are unique in that we have a Practice Educator to support our staff in their continued professional development. 

We are a 7 day service and work very closely with our GP's and practices to ensure the very best of patient care can be consistently delivered for our patients in the community.

Job responsibilities

The post holder may be required to carry out other duties appropriate with their banding and competence.

Clinical responsibilities

· Contribute to the assessment of health and well-being of patients on a caseload as delegated by DN Operational Team Leader to support and improve the care of patients with long term conditions.

· Support and protect those whose mental health, health and well-being is at risk and assist the registered nurse in the ongoing assessment, planning, management and evaluation of care.

· Work within care plans established for patients, but have the ability to recognise the changes in a patient’s condition / deterioration which require intervention and understand when to refer to the registered nurse and other senior colleagues for reassessment of patient care within scope of clinical competency.

· Support the assessment and management of patient needs through initial data collection, monitoring of patients progress, feedback discussion, and documentation. Participate in care reviews with the clinical team as required.

· Ensure high standards of patient care are given and maintained and be involved in initiatives to improve patient care.

· Refer patients to the relevant professionals for specific treatment, advice and support.

· Support the OTL (Operational Team Leader)/ RGN / AP and HCA’s in risk assessment and minimisation, ensuring all incidents and near misses are reported using the datix system and participate in the investigation as required.

· Establish and maintain communication with individuals and groups on the caseload about difficult or complex matters in such a way as to overcome any problems in communication.

· Discuss with the patient any issues relevant to their health and social care needs based on health promotion and social care good practice.

· Demonstrate a good understanding of the principles of consent and ensure valid consent is obtained prior to undertaking nursing and care procedures.

· To act as an advocate for the patient and the family, including promoting and protecting their interests, privacy and rights, equality and inclusion.

· To delegate appropriately to other team members including HCA’s and AP’s, family members, and carers adhering to the NMC Delegation Accountability Tool (2018).

· Use specialised equipment requiring dexterity including Phlebotomy, Coagulation Therapy equipment, measuring blood pressures, wound dressing, and removal of sutures, clips and defined clinical competencies including the extended Nurse Associate competencies.

· Manage anticoagulation RAT programme as competency framework. To be able to recognise and respond to any abnormalities. To liaise with GP or Specialist Practitioner accordingly. To ensure the process is documented as RAT programme. Ensuring effective communication with all parties.

· Have demonstrated proficiency to administer medications, undertake medicine management training and have completed the ‘Safe Handling of Medicines’ education programme. To adhere to FPC protocols and policies relating to the control and administration of drugs, immunisation and vaccination programme, and protocols to administer Insulin, VitB12, Flu.

· Recognise, prevent and manage (where possible) areas of conflict reporting to OTL / Management Team.

· Manage verbal complaints and inform OTL / RGN when unable to resolve.

Experience

Essential

  • Minimum 2 years clinical experience whilst completing Nurse Associate trainee programme
  • Able to work effectively as a team player under appropriate supervision and as part of a multi-disciplinary team.
  • Understanding of the scope of the role of the Nurse Associate in context of the team and organisation, and how the role may contribute to service development.
  • Experience with members of the public and care providers demonstrating good customer service skills.
  • Experience of delegation and supervision of others within the role.
  • Evidence of the application of research and evidence based care.
  • Evidence of experience in teaching / disseminating knowledge and skills.

Desirable

  • Experience of working within Community Teams.
  • Post registration experience in related environment, however we would consider newly qualified.
  • Supporting mentorship of student nurses.

  • Successful completion of the Level 5 Foundation Degree
  • Associate Nurse on NMC Register
  • GCSE English Language and Maths
