Job summary
The Team Administrator will join a
team of 14 staff Nurses, Clinical Associates and Administrator colleagues.
They will provide administration support to one locality area and support
processes within our referrals hub.
The Salus ICT are a group of
professionals serving patient populations in Aldershot, Farnborough, and Fleet.
The wider teams work across health and social care and bring together their
collective experience to provide tailored support to people in need.
The Community Single Point of
Access (CSPA) is our referral access hub for community services and our role is
to coordinate incoming referrals, reviewing identified needs with a clinician and
onward referring to appropriate partner services as appropriate.

Main duties of the job
ICT
- Maintain and update patient records and meeting records, exercising high standards of data entry and good records management
- Communicate with representatives from partner organisations and coordinate information in a timely and professional manner either through e-mail, telephone, or face to face
- Coordinate, take notes, and track follow up duties for meetings within the allocated locality
- Provide administrative support to departmental meetings as required
- Use templates to ensure accurate coding for the team
- Support caseload review, referral activity and coded activity using reports provided in conjunction with clinical teams
Referrals Hub
- Process inbound referrals and complete outbound referrals, working with the on-duty triaging Nurse to ensure all referral instances are documented, all actions completed, coded appropriately and with follow up diary entries in place.

Salus is a GP Federation which was
formed by the local GP practices within North East Hampshire and Farnham. We
hold several contracts supporting primary care to deliver services on behalf of
those practices, helping them work better together and with other healthcare

- Answers incoming telephone calls to office landlines and referrals hub mobile phone
- Support coordination of team members diaries as appropriate
The role of ICT Administrator is always developing as the Integrated Care Team grows and the post holder will have the opportunity to influence and shape this development.
