Job summary
Social prescribing is a way of engaging patients in primary care with a resource which provides support within the local community. In addition it provides GPs with a non-medical referral option that can align to existing treatments to improve health and wellbeing.
People want to be able to access information and support in a setting that is convenient and familiar to them, delivered by people they trust. The Social Prescribing Link Worker (SPLW) will offer support in a clinic environment based within general practice as well at various locations within the community including the patients home.
The role will provide information and support to patients in addition to becoming the link between the patient, GP and other service providers. The role will require managing and prioritising your own caseload, in accordance with the needs, priorities and any urgent support required by individuals.
Main duties of the job
Receive&action referrals from a wide range of agencies, work with GP practices within primary care networks, pharmacies, multi-disciplinary teams, hospital discharge teams, allied health professionals, fire service, police, job centres, social care services, housing associations,&voluntary, community & social enterprise (VCSE) organisations.Providing personalised support to individuals, their families&carers to enable them to take control of their wellbeing, live independently&improve their health outcomes. Develop trusting relationships, giving people time & focus on what matters to them. Taking a holistic approach, based on the persons priorities&the wider determinants of health. To co-produce a personalised support plan to improve health&wellbeing, to introduce or reconnect people to community&statutory services.It is vital that the SPLW has a strong awareness&understanding of when it is appropriate or necessary to refer back to other health professionals or agencies.To increase the strengths&capacities of local communities,&enable local VCSE organisations&community groups to receive SP referrals. Ensure they are supported, have basic safeguarding processes for vulnerable individuals&can provide opportunities for the person to develop friendships, a sense of belonging,&confidence.
About us
The postholder will be employed by Durham Dales Health Federation (DDHF) on behalf of Teesdale Primary Care Network. The role will be across the six Practices within Teesdale PCN: Barnard Castle Surgery, Woodview Medical Practice, Gainford Surgery, Old Forge Surgery, Pinfold Medical Practice and Evenwood Medical Practice.
DDHF is a federation of 12 GP practices across Durham Dales who came together, looking for a solution to help them provide the care patients needed, share resources and ideas, and be prepared for the challenges the NHS would face in the future.The answer they came up with was a formal collaboration between the dozen practices: the Durham Dales Health Federation.
DDHFs founding principal is to work together to provide high-quality, cost effective primary healthcare services.That means providing a range of services that all GP practices in the area can use to help look after patients.
We employ a range of Clinical staff; Pharmacists, Advanced Clinical Practitioners, GPs, Additional Reimbursement Role Scheme including 3x Social Prescribing Link Workers, care coordinators and Health and wellbeing coach as well as non-clinical, administration support staff.
