Main duties of the job
The Care Coordinator will be an experienced professional who will be responsible for coordinating,
integrating and delivering care to a broad spectrum of patients. The Care Coordinator will liaise with a
broad range of medical, social and third sector services to provide effective and synchronized care to
these patients. The Care Coordinator will also work closely with discharge teams in secondary care to
reduce the incidence of delayed discharges for patients under the care of the service and facilitate care
in the patients preferred place.

integrating and delivering care to a broad spectrum of patients. The Care Coordinator will liaise with a
broad range of medical, social and third sector services to provide effective and synchronized care to
these patients.
The Care Coordinator will also work closely with discharge teams in secondary care to
reduce the incidence of delayed discharges for patients under the care of the service and facilitate care
in the patients preferred place.
The Care Coordinator will work with all patient groups, particularly vulnerable adults, to improve their
health and wellbeing by navigating them to appropriate services, supporting them to access these
services and any appropriate funding, and delivering informal counselling, advice, and support to their
activities of daily living. The service will be targeted at specific client groups, identified by local
stakeholders such as, though not exclusively; GP practices, third sector organisations, community
service providers and existing community groups. These clients will include, but are not limited to,
patients with complex long term health needs, depression, organic mental illness, and those who are
at risk of social isolation.
The Care Coordinator will also have the capability of performing minor clinical skills such as basic
monitoring and recording of vital signs, blood pressure monitoring, on behalf of the DDHF and GP
practices. These skills may be carried out in the community following appropriate training and support.
The Care Coordinator will be working at a variety of venues in the community, including patients
homes, utilising premises in agreement with stakeholders and clients. Their role will be to give
information, train and support people in their daily lives, be the link between the patient or client group
and the GP/Community Services and work with other service providers to provide the best possible
care for their clients.
The Care Coordinator will provide information and practical support; introduce people to local services
and providers; encourage people in different communities to work together, and facilitate and support
the establishment of new groups and community services; and coordinate and integrate packages of
health and social care for their clients. They will work collaboratively with the general practice teams
to meet the needs of patients, supporting the delivery of DDHF policies and procedures.
