One Health Lewisham is growing, and our NHS@Home service is at the heart of it. We're working hard to increase capacity to deliver excellent virtual care across Lewisham, and we are therefore looking for an experiencedGeneral Practitionerresponsible for remotely overseeing patients in our virtual ward, leading, guiding and advising our Senior Matrons/Matrons, and actively monitoring them through our dashboards.

Main duties of the job

You will be delivering care of the highest clinical standards, working in conjunction with the NHS@Home Clinical Lead, Operations Manager, Senior NHS@Home Matrons/Matrons and Paramedics, ensuring the overall performance of the NHS@Home Service. At all times, you will act in a manner consistent with the GMC code of conduct and appearance representing One Health Lewisham and the NHS.

Job responsibilities

You will be responsible for the delivery and provision of a high-quality NHS@Home service for the patient population of Lewisham. The role is primarily remote with periodic in person attendance for wider team/organisation meetings.

You will deliver care of the highest clinical standards, working in conjunction with the Associate Medical Director, Operations Manager, Senior NHS@Home Matrons/Matrons and Paramedics, ensuring the overall performance of the NHS@Home service. The role will involve a mix of remote clinical monitoring of patients using digital devices and technology as well as being the overall Senior Clinician answering queries from the other team clinicians.

Lead the Virtual Ward Service alongside the nurse matrons responsible for referrals, managing virtual ward beds and overseeing other members of the team.

Support the development of this new service, including the development of relevant guidance.

Provide clinician assessments for the virtual ward service in Lewisham.

The role will focus on managing patients with various acute monitoring needs, with weekly clinical advice available from a hospital Consultant.

Provide at-distance digital clinical assessment of patients in Lewisham, when required (via telephone and video appointments).

At all times, you will act in a manner consistent with the GMC code of conduct and appearance representing One Health Lewisham and the NHS.

To provide triage where required

To provide complete consultations with assessment, treatment and management of patients of all ages presenting with primary care problems

To record full clinical notes using the OHL IT systems

To complete all admin work relating to consultations

To work as part of the wider clinical team- supporting Nurse Practitioners, HCAs, and paramedic colleagues

To manage referrals of NHS@Home patients on to acute specialties, A&E or community services where required

To give education to patients on healthy living and disease prevention

To prescribe medicines in accordance with SAS formulary

To complete coding requirements on the clinical record in line with OHL requirements

To adopt a reflective approach to clinical practice, to report all incidents and work with the team to investigate incidents

To prepare response for any complaints concerning his / her work

To follow the OHLs safeguarding and vulnerable adults policy

To restrict the use of diagnostics according to the specification of the NHS@Home service

To maintain full registration with the GMC and have full medical liability insurance

Auditing clinical activity and contributing to its evaluation

Attend team meetings and other meetings as necessary

Education Training and Development:

Engage in appropriate professional development activities in order to maintain clinical competence and performance by attending OHL staff meetings and joint educational meetings

Contribute to the facilitation of a suitable educational environment in which to support the development and learning of other staff members

Undertake teaching sessions as required to staff within the multi-disciplinary team, if required

Ensure that practice is evidence based and in line with national or other given standards

Undertake mandatory and statutory training, keeping up to date with emergency procedures undertaking training as required. These will include major incident, advanced resuscitation, fire, minimal handling

Annual reviews:

Participate in own annual NHS appraisal, own annual OHL appraisal, the revalidation process as required by the GMC and support others as required.

Development of others:

Be an effective role model and resource for the UCC team and other members of the multi- disciplinary team.

  • Full registration with the GMC Appropriate qualification to practice medicine in the UK Completion of GP vocational training or equivalent (MRCGP) Registered on a ICB Principle or Performers List with no restrictions on practice as a GP Member of a medical defence organisation Not suspended or disqualified from practice or on any conditional inclusion list with any ICB.

Experience

  • Experience 1+ years working in general practice.
  • Worked with the EMIS Clinical System.
  • Delivered clinical training and or supervision of medical students / trainee doctors.
  • Experience of providing leadership to a project.
  • Experience of informing and leading quality improvements to improve outcomes for patients.

Desirable

  • Work in an organisation that has impacted on care delivery in Lewisham
  • Experience of previously working with a Lewisham health provider in a quality improvement capacity
  • Minimum of 3 years in general practice
