PHLEBOTOMY
Domiciliary Phlebotomist/Health Care Support Worker
JOB SPECIFICATION

JOB SUMMARY
To work as part of an integrated team to provide health care and support under the direction and supervision of qualified staff to patients aged over 16 registered in Southampton City
providing care to people in the community including residential care.
Carry out blood tests at the request of GP Practices
To maintain own Continuing Professional Development.
To actively participate and contribute to team working in response to service developments and the business plan.
To manage own allocated workload.
MAIN DUTIES AND RESPONSIBILITIES
To be responsible for the day to day planning of allocated workload and cover others workload during absence.
Work within the community (e.g. within a patients own home) as a lone practitioner with telephone support. Seek advice and support from a more senior colleague for matters outside sphere of responsibility, knowledge or skills.
Obtain informed consent from the patient to treatment and the sharing of information, documenting in accordance with company procedures.
Ensure patient held records are completed for each visit and that appropriate IT systems are updated daily
To record outcome of planned visit and feedback any significant changes in patients condition to relevant clinician
To work within health and safety guidelines and policies and adhere to relevant company procedures.
Adhere to relevant Policies and Procedures using the relevant risk assessment tools/process:
Lone working in the community.
Patient handling in challenging environment where the principles of best practice regarding patient handling may be compromised.
Challenging behaviour situations
Handling medical sharps.
Handling body fluids and other hazardous material and adhere to infection control policy.
PPE in line with government guideline
Escalate concerns promptly and appropriately to clinical lead.
Monitor emails to ensure you remain clinically up to date.
Call patients the day before visit so they are aware (e.g. fasting blood test.)
Make basic judgements on patients response to treatment/plans and report findings to a qualified practitioner
Communicate with patients/carers by exchanging factual information, reassurance, tact and empathy
Establish and maintain appropriate working relationships with colleagues, patients, carers and other relevant services.
Demonstrate good practice when undertaking treatment of patients
Maintain high standard of care at all times.
Adhering to relevant training when using medical equipment and consumables to ensure they are used appropriately.
Demonstrate an awareness and understanding of patient consent for sharing information and obtaining consent prior to any procedure or treatment under GDPR
To provide administrative support and functions relevant to the service.
To deliver care to patients in their own home, demonstrating a required range of core competencies and clinical skills including:
Temperature
Pulse
Blood Pressure
Oxygen Saturation
Blood Glucose Monitoring
ECG
Venesection
Be aware of own clinical boundaries, both in daily practice and in emergency situations.
Acknowledge and recognise peoples expressed beliefs, preferences and choices promoting equality and diversity
Demonstrate an awareness of clinical governance and risk management, and apply to working practices.
Comply with Company Clinical Governance and Health and Safety requirements within agreed policies and procedures.
Follow and contribute to new policies and local guidance and contribute comments on proposed/required changes to meet changing service needs.
Assist with maintaining a confidential filing system for patient records and other relevant information.
Undertake routine administrative tasks e.g. photocopying, filing within current company policies and procedures as required by the service.
Defective equipment is reported and made safe without delay in accordance with company procedures.
PROFESSIONAL DEVELOPMENT
Maintain own continuing professional development.
Complete mandatory training as required in development of job role.
Adhere & complete Venesection training, working towards completion of competency framework within agreed timescales.
Meet regularly with the line manager to reflect on performance and discuss any future development needs.
MANAGEMENT AND LEADERSHIP
May not suspend or dismiss staff under the disciplinary procedure.
May not take annual leave without the prior agreement of the manager.
May not undertake nursing tasks without appropriate training and education appropriate to role.

COMMUNICATION
- To support effective communication.
- Utilise and demonstrate sensitive communication styles, to ensure patients are fully informed and consent to treatment.
- Communicate effectively with patients and carers, anticipating and recognising the need for alternative methods of communication to overcome different levels of understanding, cultural background and preferred ways of communicating.
- Ensure awareness of sources of support and guidance and provide information in acceptable format to all patients, recognising any difficulties and referring where appropriate.
- Maintain effective communication with Service manager, colleagues and other providers
GENERAL RESPONSIBILITIES
- The post holder will ensure adherence to Company Policies and Procedures at all times

- Employees must be aware of the responsibilities placed on them under the Health & Safety at Work Act (1974) and the Manual Handling Operations Regulations (1992). This ensures that the agreed safety procedures are carried out to maintain a safe environment for employees, patients and visitors to SMS.
