Job summary

Bristol Inner City Primary Care Network (BIC PCN) are looking to expand its Clinical and Operational teams, working at Charlotte Keel Medical Practice (CKMP) within our diverse and dynamic inner city Primary Care Network.

You will be working collaboratively within Charlotte Keel Medical Practice, alongside both the GP, nursing and admin staff, to meet the needs of the patients by providing direct care and support using established guidelines

About us

You will work as part of the BIC PCN, a forward thinking group of 7 surgeries comprising of Broadmead Medical Centre (BMC), Charlotte Keel Medical Practice (CKMP), Homeless Health Service (HHS), Eastville Medical Practice, Lawrence Hill Health Centre, Montpelier Health Centre and Wellspring Surgery.

Job responsibilities

Clinical

Mental Health Practitioner - This new role of Specialist Mental Health Practitioner for primary care is fundamental to the development and leadership of innovative new teams and integrated mental health services. This role promotes and provides early assessment, treatment and improved access to evidence-based interventions supporting people contacting primary care for help with their mental health needs.

Physician Associate - is a trained healthcare professional who works directly under the supervision of a doctor as part of the medical team. They are usually generalists with broad medical knowledge but can develop expertise/specialisms in a particular field.

Clinical Pharmacist - working as part of a multi-disciplinary team in a patient facing role to clinically assess and treat patients using their expert knowledge of medicines for specific disease areas.

Advanced Practitioner - You will provide care for the presenting patient from initial history taking, clinical assessment, diagnosis, treatment and evaluation of care.

First Contact Physio - you will provide expertise, acting as first contact physiotherapist and making decisions about the best course of action for patients' care.

Operational

Care Coordinator - play a pivotal role in proactively identify and work with people, including the frail/elderly and those with long term conditions to provide coordination and navigation of care and support across health and care services.

GP Assistant - is an enhanced administrative role, with some basic, clinical elements. Supporting the smooth running of clinics by performing the more routine administration and basic clinical tasks on behalf of the GP.
