Job summary

The Homeless Health Service provide Primary Healthcare for some of the most vulnerable and challenging patients in our community. BrisDoc are looking for a Lead Nurse who shares our passion for Patient care by people who care to join our team at the Compass Centre in Bristol.

The Homeless Health Service is based in the Compass Centre, but also run a number of outreach clinics at locations such as the Wild Goose Caf, One25, local hostels and Bristol Drugs Project.

This post forms part of the Clinical Leadership Team, and the post holder will therefore work closely with the Lead GP, Service Manager and the Deputy Medical Director in strategically and operationally leading the Service forward, achieving quality outcomes for patients and maintaining a high success rate in the Enhanced Services and Key Performance Indicators.

Salary: please note that the advertised salary is PA, pro-rata.

Hours:32 hrs/wk to include both clinical and management time, with management time approximately 40%.

Main duties of the job

As Lead Nurse, you will be part of a proactive team of experienced professionals and you will be involved with the future design and development of this and other services.

You will be providing excellent healthcare in a variety of forms to our complex client group, reporting to the Lead GP for the Homeless Health Service.

You will be expected to split your time between leadership and supporting colleagues in covering nursing shifts. This will be flexible based on the needs of the service at any particular time.

  • To provide professional leadership to the nursing team
  • To organise and facilitate the day to day running of Compass Health in conjunction with the Service Manager and Lead GP
  • To stimulate an awareness of client needs and influence policies affecting the health of the homeless

The Homeless Health Service (HHS) is being delivered from The Compass Centre in Jamaica Street,Stokes Croft, Bristol. The health centre is in a shared building with dedicated rooms and controlled access. The surgery can register up to 100 patients but predominantly runs as a drop in centre. The service is open five days per week and provides GP and nurse appointments,

The patients seen in the HHS often have very chaotic lives, many having mental health illness, alcohol dependence or drug addictions. Staff working at the HHS will need to have experience in these areas.

Job description

Job responsibilities

  • Work closely with the Lead GP, meeting regularly to plan the clinical aspects of the service delivery and to identify areas of improvement or development.
  • To network and engage with external stakeholders to develop cross agency working and to help shape the strategic development of the service.
  • Provide professional support and advice to all Homeless Health Service staff
  • Manage the primary health care nursing team
  • Work as a specialist nurse within the team to provide direct clinical care to homeless adults with complex needs, including assessment, nursing care, prescribing, referral, sign posting and facilitating health enhancing activities
  • Participate in the teaching of medical and nursing students and training of staff, volunteers and service users
  • Be responsible for the strategic and operational management of the nursing team, the implementation of nursing strategies to support the care of our patients, and the continued improvement and development of nursing practice
  • Have key management responsibilities include communication with the nursing team, team development, rota management, absence management, performance and development reviews, induction, professional training and development and recruitment and selection of new team members. They will work together with the Service Manager and Lead GP to ensure that all areas of the practice are considered and the practice as a whole is holistically managed.
  • Ensure systems at the Compass Centre and all outreach sites are in place for managing and auditing compliance for the Care Quality Commission (CQC) to achieve the highest ratings with support from the Management team, the wider BrisDoc team, the infection control and health and safety leads and other relevant leads within the practice / BrisDoc.

Shifts will be rostered on a rolling 4 week basis and may include some weekend working (rare). Management time will be scheduled within the structure of the day. As HHS service expands, the management/clinical function undertaken by the post holder will reflect the needs of the service.

Clinical Governance:

  • Contributing to the development, ongoing review and implementation of key policies
  • Significantly contributing to the Risk Management and monitoring of clinical incidents
  • Participating in the investigation of clinical complaints and significant events in conjunction with the Service Manager and Lead GP
  • Overall taking responsibility to ensure that the nursing team are functioning at a good clinical standard.
  • Coordinating medicine management and monitoring of key clinical equipment.
  • Ensuring the nursing team are using the correct EMIS clinical codes for maximizing KPIs
  • Oversee the cascading and actioning of patient safety alerts.
  • Work closely with safeguarding leads to ensure a robust system for monitoring and managing children and vulnerable adults.

Clinnical Responsibilities:

  • Assess, diagnose, plan, implement and evaluate treatment/interventions and care for patients presenting with an undifferentiated diagnosis
  • Clinically examine and assess patient needs from a physiological and psychological perspective, and plan clinical care accordingly
  • Assess, diagnosis, plan, implement and evaluate interventions/treatments for patients with complex needs
  • Developing management plans, support and review programmes to help patients effectively manage their long term conditions.
  • Prescribe and review medication for therapeutic effectiveness, appropriate to patient needs and in accordance with evidence-based practice and national and practice protocols, and within scope of practice
  • Work with patients in order to support compliance with and adherence to prescribed treatments
  • Assess, identify and refer patients presenting with mental health needs in accordance with the NSF for Mental Health
