Job summary
This role involves providing operational and strategic support to the Primary Care Network (PCN) to enable delivery of the NHS England Primary Care Networks DES.
CORE HOURS
- Full time, 37.5 hours per week.
- Occasional evening meetings will be required.
- Flexibility is a requirement of the position in line with service need
LOCATION
- The role will involve working at the PCN main site which is at Primary Healthcare Darlington (GP federation), Forsyth House, 20 Woodland Road, Darlington, DL3 7PL.
- There will be flexibility around working from home.
Main duties of the job
Working alongside the
PCN’s Executive Board members, the post holder will play an integral role in
developing and implementing the systems between the PCN’s practices, alongside
the local health providers and voluntary community organisations.
The role requires the
post holder to have excellent leadership and communication skills, and the
ability to be self-directed and prioritise.
The post holder will provide a high
level of strategic, operational and project management.

Darlington Primary Care Network is a working collaboration of 11 general practices with a combined patient population of over 110,000 patients, making Darlington PCN one of the largest PCNs in England. We have close links with Darlington Borough Council and local NHS Foundation Trusts and have the full support and backing of Tees Valley CCG.

Support to the PCN Clinical Director and Board:
- Supporting the provision of strategic and clinical leadership to the PCN including developing and implementing strategic plans.
- Supporting quality improvement and performance across the practices for areas as set out in the PCN DES and for other contracts.
- Supporting the strategic leadership for workforce development.
- Supporting network implementation of agreed service developments and pathways, working closely with member practices, other local PCN’s and the local system to deliver national and local priorities.
- Attending meetings as required on behalf of the Clinical Director or the PCN and feeding back to the PCN members.
- Prepare and annually update PCN strategic documentations i.e. the Network Agreement, Business Plan, Annual Report, Communications and Marketing Strategy and Digital Strategy.
- Regular reporting to the CD and Board on service provision and development which is aligned to the aims and objectives of services as agreed by the Board.
Financial Management:
- Administration and reconciliation of financial transactions and maintaining accurate financial records.
- Preparing regular PCN budget reports to the PCN Board including actual and forecast income and expenditure.
- Contract management for commissioned services with external organisations to maximise value for money.
- Producing business cases and other funding bids for external funding.
- Preparation of annual financial reporting to meet the requirements of the PCN’s financial framework.
Service Management:
- Effective management of contracted services where the practices or PCN is the provider i.e. Locally Commissioned Services (LCS).
- Effective management of commissioned services with partner services to ensure value for money and efficient and effective service provision i.e. ARRS roles.
- Maximisation of service achievement against the PCN DES including the Investment and Impact Fund framework.
- Managing the PCN maturity matrix and other essential requirements for reporting to the Integrated Care System (ICS)/NHSE.
- Regular review of service performance to inform service development planning.
- Oversee the development of communication and marketing products to proactively promote PCN and practice services and best practice.
Project Management:
- Provides leadership in the delivery of PCN projects.
- Ensures effective planning and reporting of PCN projects.
- Ensures timely and effective project outcomes for the delivery of successful business as usual products.
Working Relationships:
- Is able to gain the confidence and buy in from member practices to enable their full participation in networks and their delivery.
- Acts as a key PCN representative at senior level.
- Supports public engagement and consultation on PCN planning.
- Develops relationships and works closely with other PCN’s, Federations, clinical leaders of other health and social care providers.
- Provides leadership in supporting staff, public and stakeholder engagement.
- Provides partnership working with Public Health to identify gaps and shared solutions to issues facing primary care and ensuring that patient voice is heard.
People Management:
- Provides supportive and effective leadership for PCN support roles
- Delegates clearly and appropriately, adopting principles of effective programme and project management.
- Is proactive in encouraging staff to engage in professional development opportunities.
- Successfully manages recruitment or commissioning processes of workforce to deliver against PCN workforce strategy and business plans.
Governance:
- Ensures compliance with CQC regulations.
- Effective management and governance of the PCN’s financial framework.
