Job summary

We are looking for enthusiastic and committed individuals who would like to pursue a career in primary care, in a developmental role as a Trainee Nursing Associate (TNA) who would qualify to be a Registered Nursing Associate (RNA). Within the apprenticeship period, you will study with Teesside University, working towards the NMC standards of proficiency for Nursing Associates. You will combine your academic work with on-the-job training, within an exciting and innovative primary care setting in Darlington PCN and deliver care, based on the needs of the population of Darlington.

The TNA foundation degree training programme is usually completed over two years. During this time, you must complete at least 2,300 programme hours, which are divided to achieve an equal balance of theory and practice learning.

To meet the requirements of the training programme, TNAs must work in a range of settings and situations to gain as much experience as possible across different age groups. You must complete external placements, totaling 460 hours across 2-years, in settings other than your primary place of employment, which will be a GP surgery in Darlington.

Primary Healthcare Darlington was established in 2014 as the local GP federation consisting of 11 GP Practices covering an ever-increasing population of 108,000+ registered patients in Darlington, County Durham. We have close links with Darlington Borough Council and local NHS Foundation Trusts and have the full support and backing of Tees Valley ICB. We also work closely with and provide support to the Darlington Primary Care Network.

Working from newly refurbished dedicated premises close to Darlington Memorial Hospital we provide NHS, Public Health and Private Medical Services.

Job responsibilities

We are looking for highly motivated, enthusiastic, and resilient individuals to undertake a 2-year level 5 apprenticeship ending in registration with the NMC as a Nurse Associate. As a Trainee Nursing Associate Apprentice, you will gain experience on clinical placements across a range of specialities, covering the four fields of nursing. You will work alongside Registered Nurses and other clinical professionals to develop the skills and knowledge required to complete your Trainee Nursing Associate apprenticeship and to ensure the delivery of outstanding care to our patients.

Entry requirements: you should be highly motivated towards a career as a Nursing Associate within the primary care setting. You should have excellent communication skills and be able to demonstrate evidence of GCSE A-C/4-9 or functional Maths & English level 2 and a full UK/EU clean driving licience.

Providing and monitoring care

On-going monitoring of care through dementia reviews, mental health reviews, hypertension checks and new patient checks

Being aware of clinical guidelines and signs and symptoms of deterioration, escalating concerns to appropriate colleagues for them to reassess and evaluate the care.

Escalation of concerns

Gathering of social, medical and socioeconomic information to contribute to the care process

Display skills in effective communication across the age continuum

Effective team working and relationship building skills with both colleagues, patients/clients and their families and carers

Supporting individuals and their families and carers when faced with unwelcome news and life-changing diagnoses

Ensuring the privacy, dignity and safety of individuals is maintained at all times

Recognising issues relating to safeguarding vulnerable children and adults

Monitoring changes in people's ability to carry out activities of daily living and mental & physical health such as a deterioration in their long-term condition

Monitoring of vital signs; skills in venepuncture, manual blood pressure and pulse, oxygen saturations, foot checks; wound care and infection control measures such as aseptic technique

Delivery of immunisation and vaccination programmes alongside the nursing and wider clinical teams.

Working in Teams

The role will work across the continuum of primary care services, based mainly in the 11 practices of Darlington PCN working with GP teams, reception teams, Primary Healthcare Darlington team, community and district nursing service; mental health services through Tees, Esk and Wear Valley Trust; this list is not exhaustive.

Once qualified you will support and supervise trainee nursing associate apprentices, health care support workers and those new to care roles, for example, those undertaking the care certificate with their learning and development.

During your apprenticeship you will be supported by the TNA programme lead, the PCN Practice Nurse Lead and other registered nurses you work with, as well as HCAs, GPs, Pharmacists and the wider primary care teams clinical and non-clinical.
