Job summary

The care home pharmacy technician will support the Enhanced Health in Care Homes team by working with care homes and GP practices to deliver a programme of medicines optimisation. This work will include reviews of care home residents medication, facilitating safe transfer of residents into and out of care homes in respect of medications and assisting GP practices to provide safe and appropriate quantities of medication to care home residents. The work will compliment clinical pharmacists, community pharmacists and other members of the PCN multi-disciplinary team.

Main duties of the job

able to undertake the following responsibilities:-

  • Synchronisation of medication for new residents into a care home to ensure all quantities match for 28 days supply.
  • Align new residents with the care home ordering cycle.
  • Work with Community Pharmacies supplying care homes to sort out supply issues and identify medication not mapped to ETP, which is generating paper scripts.
  • Work with care homes to amend quantities depending on patient needs and to address any medicine shortfalls
  • Correct any directions that need amending that are out of date titrating instructions, unclear or ambiguous.
  • Add medicines that are not supplied by the Practice as other medication to pick up any interactions with it.
  • Assist with education & training for GP reception staff as well as assisting with any ad-hoc care home resident medicine queries and tasks.
  • Liaise with GP staff to assist with clinical correspondence updating any changes to medication for care home residents

Job Title: Extended Health in Care Homes (EHICH) Pharmacy Technician

Location: Based at Forsyth House in Darlington with the requirement to work in one or more GP practices and in one of more Care Homes in the Darlington Primary Care Network

Responsible to: Enhanced Health in Care Homes Lead GP

The post holder will be responsible for encouraging the development of better understanding of the principles of medicines optimisation in relation to care home residents throughout the practice teams and promoting good practice in line with therapeutic developments.

Key responsibilities:

Synchronisation of medication for new residents into a care home to ensure all quantities match for 28 days supply.

Align new residents with the care home ordering cycle.

Check Pharmacy who supply care home and amend as needed.

Work with Community Pharmacies supplying care homes to sort out supply issues and identify medication not mapped to ETP, which is generating paper scripts.

Work with care homes to amend quantities depending on patient needs and to address any medicine shortfalls

Correct any directions that need amending that are out of date titrating instructions, unclear or ambiguous.

Add medicines that are not supplied by the Practice as other medication to pick up any interactions with it.

Identify residents with a weight under 50 kg to check the dose of paracetamol prescribed.

Review calcium intake to assess whether calcium and vitamin D are still needed for them.

Assist with proxy ordering work.

Assist with ordering pilot work.

Assist with education & training for GP reception staff as well as assisting with any ad-hoc care home resident medicine queries and tasks.

Liaise with GP staff to assist with clinical correspondence updating any changes to medication for care home residents

Contribute to education of care home staff

Where appropriate, assess patients ability to take medicines as prescribed i.e. checking inhaler technique and making recommendations to the medicines optimisation pharmacist to address any issues identified.

Undertake Medicines reconciliation, checking medicines dispensed and administered against Medicines Administration Record charts and GP practice records.

To support investigation of medication related incidents

To support the implementation of NPSA alerts in relation to medicines

Able to plan and work independently and manage competing priorities

Able to plan own work with a high degree of motivation

Pharmacy Technician NVQ Level 3

Previous Primary Care, Care Homes or Frailty experience

Comprehensive experience of Systm1 clinical system

Computer Literate

Effective interpersonal skills

Excellent communication skills both verbal and non-verbal

Specific requirements:

A satisfactory Disclosure and Barring check

Ability to travel between service sites

Completion of CPPE pharmacy technician pathway or willingness to undertake the pathway

Registration with the General Pharmaceutical Council / immediate requirement to register with the GPC
