Job summary

We are looking to recruit Health Care Assistants to join our friendly team to provide Enhanced Access Services for our Primary Care Networks in North Manchester & Ancoats Primary Care Centre on Saturday 9.00am - 5.00pm. The Enhanced Access Service is available to registered patients from various Primary Care Hubs to delivering high quality health care appropriate to the needs of the local population and provide an extension of their own GP Practice service outside of core hours.

You would work alongside other health care professionals and be supported by a reception team with on call support.

We would welcome applicants that are experienced, friendly, flexible and can work as part of a multi-disciplinary team.

EMIS Web experience is essential

Job description

Job responsibilities

Northern Health GPPO Ltd is located in the North region of Manchester to provide extended Primary Care Access to its patients registered within 33 local GP practices. The service is available to registered patients during the weekend and weekends from various community based primary care hubs, strategically placed across the catchment area to provide equity of access.

The service provides patients with access to a GP, a Practice Nurse or HCA via a pre-booked appointment for planned or unscheduled presentations as an extension of their own surgery outside of normal surgery hours.

The Extended Access Primary Care HCA will work alongside and support the service based primary health care team, delivering high quality health care appropriate to the needs of the local population. You will be supervised by all members of the nursing team, working within agreed protocols.

Main /Clinical Duties

New registration health checks

Height /weight/body mass index assessments

Basic medical history

Health Promotion including advice on diet, exercise, smoking cessation Sexual Health advice.

Cytology Screening advice

Breast awareness advice

Smoking cessation sessions as intermediary advisor

Blood pressure monitoring

Venepuncture

Assist with routine screening of all asthma, COPD, diabetic and IHD patients

Assist with Spirometry under supervision of primary care nurses

Urinalysis

ECG recording

Act as chaperone/assist with clinical examination as required

Act as patients advocate promoting patients point of view to others

Maintain accurate records of patient care in electronic format using correct codes

Report changes in condition of patient/areas of concern to relevant member of clinical staff and ensure documented action is taken

To follow relevant protocols, guidelines and best practice, e.g. local protocols, NICE guidance

General Responsibilities

To contribute to safe working environment for all through adherence to health and safety management systems

Correctly handle confidential information in line with your duties, and to actively support information governance policies, processes and systems

Support the services approach to equal opportunities, acting in line with the services equality and diversity policy

Participation in the services appraisal and personal development processes

To work at all times within boundaries of ones competence, taking personal responsibility to keep abreast of change

To act in a professional manner at all times towards patients and staff

Any other duty as requested, including covering for absent colleagues
