Job summary
An exciting opportunity has arisen for an experienced practice nurse to join a well established and thriving GP Practice in North West London on a part time basis (3 days a week)
Main duties of the job
The practice nurse will work as part of an excellent
administrative and multidisciplinary clinical team to provide high quality
nursing care to patents. They will play a key role in patient education and
supporting self-care.
Providing and evaluating a range of health
interventions to ensure that patients health and social care needs are met.
Health and well-being promotion plus
individual person-centred plans for patients with/at risk of a long-term
condition.
Managing and treating long-term conditions in
line with national and local policies and practice needs.
Reviewing medication for therapeutic
effectiveness, in accordance with evidence-based practice and national/practice
protocols and work with patients to support adherence to prescribed treatments.
Prioritising health problems, intervening
appropriately, to assist the patient in complex, urgent or emergency
situations, including initiation of effective emergency care; Supporting and
managing health needs of women presenting for family planning and cervical
cytology consultations.
Implementing and participating in vaccination
and immunisation programmes for both adults and children including patients
travelling abroad
Meeting the needs of patients presenting for
opportunistic wound care.
Hypertension review and management; Running
Asthma, COPD, Diabetes clinics and reviews.
New patient and health checks.

Care Ltd. It currently cares for 9,000
patients living predominantly in Brent and Ealing Boroughs. The practice is part of Harness South Primary
Care Network (PCN) and Harness Care Ltd provides support to both Harness South
and Harness North PCNs. The combined
populations of the two PCNs is 150,000 patients providing a range of

- A relevant nursing degree and at least 2 years post qualification experience in a General Practice setting
- Current NMC registration with an active PIN
- Fundamentals of Practice Nursing course (General Practice Nurse Foundation Programme) or equivalent experience or qualifications
