Harlow North Primary Care
Network are a group of five Practices within Harlow. We jointly
deliver services that improve outcomes and provide positive patient experience
for our population of 60,000+.
As a Primary Care Network (PCN) our aim is to
work collaboratively, enabling our team of Social Prescribers, Care
coordinators, Clinical Pharmacists, Nursing Associates and GP Assistants to
work together with the Practices. We seek continuous improvement by
strengthening our relationship with other key health and wellbeing providers to
maximize the impact of our services on quality and outcomes of patient care.
The Practices within the PCN
are Addison House, Church Langley, Nuffield House, Sydenham and Old Harlow All
practices use Systmone clinical system.
We are a friendly, supportive, democratic, well organised and
enthusiastic Primary Care Network. We contain training practices and actively
encourage learning and development for all staff.
Main duties of the job
The
Transformation and Development Manager will work with clinical and support
teams within a Primary Care Network (PCN) to identify opportunity for and lead
service improvements.
The post holder will plan, coordinate, and directly support delivery of evolving
projects across the PCN with a view to improving patient access, staff and
patient satisfaction, optimising the use of technology and improving the
efficiency and sustainability of general practice services.
They will work closely with staff working at practice and PCN
level to understand current service models, identify challenges, agree
priorities for improvement and then support these to be delivered. They will develop strong relationships with PCN member practices and will
support the planning and delivery of improvements that will provide benefits
both practice and PCN level.
The post holder will work with the ICB Primary Care and
Digital teams to ensure local transformation plans and activity are aligned to
the ICB Primary Care priorities e.g. improving access, PCN development and
the ICB Digital Strategy.

required to deliver transformation plans, e.g., in analytics, evaluation,
policy, IT systems, estates etc.
The post holder will help to develop strategy related to transformation
and improvement, aligning this with larger scale strategy and initiatives; but
will focus their time on the planning, implementation and evaluation of
practical improvements that make a difference to patients and staff, ensuring
they address health inequalities and championing diversity and inclusion.
