Job summary

Job summary

· The post holder will manage a caseload and deal with a wide range of health needs in a GP Practice setting, ensuring the highest standards of care for all patients referred into the service.

· To provide practice pharmaceutical advice and to promote safe, effective and evidence based cost effective use of medicines within the practices /hubs.

· To prevent, detect and address medicines-related problems and to achieve optimum use of medicines within the practices /hubs.

• Conduct targeted holistic medication reviews with patients.

Main duties of the job

Be an effective member of the GP multidisciplinary teams.

Resolve queries, complete medicines reconciliation, arrange any monitoring and read code accordingly.

Review complex patients having care provided by multiple agencies, e.g. patients in care home, at the end of life.

Identify acute prescriptions generated for medications for long term conditions. Conduct face to face reviews with patients on multiple medications supporting patients/carer

Support the GP Practice team to develop a ‘risk stratified’ approach to health and care provision, in which services are tailored to clinical need and level of risk of individuals and meet the demands on the practice.

Implement, and monitor the use of medicines related ‘risk stratification’ processes

Provide regular clinical updates to practice staff on drug updates, safety alerts and medicines related NICE guidance and support implementation of recommendations in practice.

Support the improvement of medicines related processes within the practices.

Manage the care of patients on medicines with chronic diseases.

Maintain a good relationship with partners and other local pharmacists. Medication reviews where adherence or other issues have been identified.

Carry out the duties in a diligent manner and to the best of their ability and use their best endeavours to promote the interest of the patient

• Identify acute prescriptions generated for medications for long term conditions and follow up patients.

• Record significant events relating to medicines and support practice and system wide learning and improvement.

• Conduct face to face reviews with patients on multiple medications supporting patients/carers:

• To understand their medicines and health condition and the effect on their body.

• To participate in decision-making about their medicines use and their care

• To make informed choices about their treatment, understanding the risks and benefits.

• To understand the need to make necessary changes to their lifestyle for managing their condition.

• To be able to ask questions of the healthcare professionals providing their care in all settings.

• To take responsibility for their health and medicines and to know how and where to actively seeks advice /care when necessary

• Support the GP Practice team to develop a ‘risk stratified’ approach to health and care provision, in which services are tailored to clinical need and level of risk of individuals and meet the demands on the practice.

• Implement, adapt and monitor the use of medicines related ‘risk stratification’ processes

• Integrate pharmaceutical health and care services liaising with community, hospital and other pharmacists within the system to improve patient experience and to clarify and reduce the patient safety risk associated with medicines use at transfers of care in care pathways and processes.

• Share educational resources and hold training events for practice staff about key national and local issues related to medicines use.

• Provide regular clinical updates to practice staff on drug updates, safety alerts and medicines related NICE guidance and support implementation of recommendations in practice.

• Support the improvement of medicines related processes within the practices / hubs; e.g. repeat prescribing, near patient testing and high risk drug monitoring.

• Work with the patient participation groups to support patient empowerment and best use of medicines.

• Optimise use of technology, to improve practice / hub effectiveness and safety and to promote patient independence and self-management.

• Take an active role in the adoption of new models of provision or the development of new service.

• Manage the care of patients on medicines with chronic diseases. This includes using the clinical computer systems for input of information. Accurate and efficient input of data for GMS contract purposes may also be required.

• Meeting patients’ care needs as identified, or where care has been transferred by a GP.

• Maintain a good relationship with partners and other local pharmacists, to secure ongoing support of and adherence to safe, consistent, transparent and seamless medicines governance processes

• Manage all existing and emerging medicines related interface queries and issues promptly to ensure patients receive treatment from the clinically appropriate prescriber, participating in the redesign of primary and secondary care service pathways in conjunction with other partners where necessary

• Ensure that patient representation is included within local medicines processes and that patient support material is available as required.

• Implement evidence based medicine and currently accepted best practice in GP practices

• Initiate and maintain health screening and health promotion clinics for identified areas of the West Essex population, recognising where modification of systems or referral is necessary.

• As and when required, manage all or some of the following conditions and services:

• Asthma

• Type 1 and Type 2 Diabetes

• Hypertension

• Pain

• COPD

• Anticoagulation

• Atrial fibrillation

• Vulnerable children and adults (including care homes and housebound patients)

• Patients with learning disabilities

• Maintain efficient liaison with all areas of the Hub Team and other agencies as required in order maintaining continuity of care as appropriate.

• Support routine treatment in general clinics, including:

• Anticoagulant / high risk medication monitoring

• Medication reviews where adherence or other issues have been identified.

• Immunisations

• Support health promotion programmes by providing a holistic assessment and lifestyle advice on diet, smoking, alcohol intake and exercise.

• Assess problems presented opportunistically by patients; dealing with minor illness.

• Ensure awareness of statutory and local clinical protection procedures, including systems of referral. Ability to recognise signs and symptoms of child abuse.

• Be able to recognise and manage anaphylaxis according to current guidelines.

• Be able to perform Cardio-pulmonary resuscitation according to current UK guidelines.

• Carry out the duties in an expert and diligent manner and to the best of their skill and ability and use their best endeavours to promote the interest of the patient

• Notify the Practice Manager, GP Prescribing Lead and Stellar Healthcare Medical Director of any untoward incident or accident which occurs whilst on Practice premises. An accident / incident form must be completed in accordance with Practice Policy
