Job summary
Primary Care Network 4 (PCN 4) are expanding there team and are looking for a Health & Wellbeing Coach to join their friendly, dynamic and progressive PCN. The successful candidate will be based across Mill Hill and Edgware in West Barnet supporting 4 practices.
*We are also recruiting for Maternity cover*
PCN 4 consists of 4 GP practices serving a diverse population of over 50,000 patients. We have excellent collaborative working relationships. Our practices are high-achieving and innovative training practices, and our focus as a PCN is to provide high quality, holistic patient care as well support to our practices.
You will need a background in supporting individuals to achieve their health and well being goals!
Wellness coaching' can be defined as the application of life-coaching principles to health and wellbeing. Our Health and Wellbeing coach will use skilful communication to support individuals to make life changes through working towards their personal goals
The role would suit an individual that meets the below criteria:
- Do you have a background in empowering people to overcome barriers and achieve lifestyle improvement goals
- Do you have an understanding of the psycho-social factors that may limit an individuals well being, including mental health awareness?

Main duties of the job
- To coach and motivate patients through multiple sessions to
identify their needs set goals and support them to implement their
personalised health and care plan.
- To provide personalised support to individuals, their
families, and carers to ensure that they are active participants in their
own healthcare and to empower them to take more control in manging their
own health and well-being, to live independently and to improve their
health outcomes through the following:
- Providing interventions such as self-management, education,
and peer support
- To actively signpost patients to the correct healthcare professional.
- Supporting people to establish and attain goals set by the
person based on what is important to them, building on goals that are
important to the individual.
Working with the
social prescribing service to connect them to community-based activities which
support their health and well-being.

Barnet Federated GPs is a rapidly growing GP Federation whose shareholders include all 48 practices in Barnet, covering over 425,000 patients. Within this, we are divided into 8 Primary Care Networks (PCNs), each with its own leadership structure. Barnet Federated GPs is made up of a small, office-based team along with some clinical and non-clinical staff whom we employ on behalf of the PCNs. We currently have an enthusiastic and very supportive Board of Directors leading Barnet Federated GPs who are focused on supporting and sustaining Primary Care in the London Borough of Barnet.

Essential
- Educated to GCSE level or equivalent.
- GCSE Mathematics & English (C or above)
- Enrolled in, qualified or willing to undertake training from a health coaching provider accredited by the personalised care institute
- Enrolled in, qualified or willing to undertake training to become a stop smoking advisor
Desirable
- Level 3 accredited Personal Trainer award.
- Relevant qualification in health and wellbeing/physical activity or nutrition
- Qualified/certificate in Psychological/mental health First Aid
- Counselling or coaching skills-based qualification
- NCSCT certified stop smoking practitioner
Experience
Essential
- Experience in a Physical Activity / Sports Development/ Health area within the public, voluntary or private sector
- Experience of working with groups
Desirable
- Experience of health promotion/health behaviour change work with individuals and community groups
- Evidence of a CPD portfolio
- Skilled in the use of a range of service improvement tools
- Experience with alcohol or drugs intervention
- Working in partnership with professionals from a range of backgrounds
- Broad knowledge of general practice
- An appreciation of the new NHS landscape including the relationships between individual practices, PCNs and the commissioners.
