Job summary
Barnet Federated GPs is collaborating with Haringey GP Federation to provide the NCL General Practice Change Management Contract.
This is a new and exciting hybrid role working across North Central London (Barnet, Camden, Enfield, Haringey & Islington) working with individual GP Practices, and potentially PCNs, across NCL to provide hands-on change management, quality improvement expertise and capacity to make change happen.
You will support practices to make meaningful changes to their operating models and ways of working to deliver sustainable improvements to the functioning of the practice and improved services and care for patients. The staff involved in the service will be a mix of clinicians and non-clinical staff with Quality Improvement experience who have a good blend of technical, interpersonal, and organisational skills, understanding of and credibility with general practice.

Main duties of the job
The Clinical Facilitator will play a key leadership role in supporting the delivery of the NCL General Practice Change Management Support Programme at the borough and practice level.
They will act as the clinical interface between GP Practices, PCNs, Federations, and the central programme team, ensuring clinical credibility, alignment to local priorities, and engagement from frontline staff.
Working alongside Business Change Facilitators and Borough Senior Managers, they will support practices to embed improvements in Access, Long-Term Conditions (LTC), and Digital Transformation, ensuring that all changes are clinically robust, patient-centred, and sustainable.
The postholder will champion quality improvement (QI) approaches, support peer learning, and help practices apply evidence-based models of care to achieve better outcomes for patients and staff.

Barnet Federated GPs is a rapidly growing GP Federation whose shareholders include all 48 practices in Barnet, covering over 425,000 patients. Within this, we are divided into 8 Primary Care Networks (PCNs), each with its own leadership structure. Barnet Federated GPs is made up of a small, office-based team along with some clinical and non-clinical staff whom we employ on behalf of the PCNs. We currently have an enthusiastic and very supportive Board of Directors leading Barnet Federated GPs who are focused on supporting and sustaining Primary Care in the London Borough of Barnet.

- Qualified GP, Nurse, Pharmacist, or Allied Health Professional with current registration.
- Significant experience working in General Practice or Primary Care Networks (PCNs).
- Demonstrable leadership or facilitation experience in clinical improvement or transformation.
- Understanding of QI and change management methodologies (e.g., PDSA, Kotter, Lean).
- Ability to engage and influence peers and clinical teams.
- Strong communication, facilitation, and relationship-building skills.
- Awareness of current NHS priorities and system working within an ICB/ICS context.
Desirable
- Previous involvement in system-level improvement or transformation programmes.
- Experience in coaching or mentoring clinicians and practice teams.
