Job summary
This is an exciting opportunity to support your local Training Hub team in providing training and practical support to local GP practices to prepare for the new Long-Term Conditions (LTC) Locally Commissioned Service (LCS).
The post holder will be responsible for championing the model of care and providing training & and clinical guidance to GP practices within the borough for the new LTC LCS. This will include co-ordinating condition specific training requirements, Personalised Care & Support Planning, and a wellbeing tool (MyCaW).
The role will work closely with borough Business Change & Quality Improvement Facilitators as well as support from the wider Training Hub team

Main duties of the job
1.1.Practice Readiness Checklist - Provide support and/or training to practice staff on the Model of Care, the desktop review process, the wellbeing tool and field clinical specific questions
1.2.Personalised Care & Support Planning - Provide accredited Personalised Care & Support Planning support and/or training to primary care providers (following internal train-the-trainer training to be provided if required)
1.3.Condition specific training Work with the 4 other borough Clinical Facilitators and NCL Clinical Lead to identify frontline training priorities & support provision if required
1.4.Community of practice Work with the Training Hub team to continue to develop a community of practice for Long Term Conditions related work, including active signposting into other networks and Quality Improvement Methodology
1.5.Targeted support Work with borough-based Training Hub team to identify and respond to queries from practices that require additional support to implement LTC LCS

The NCL Training Hub has been established to enhance training, education, and workforce development with specific focus on enhancing the recruitment of new, and retention of current, staff. The NCL Training Hub has developed innovative educational and workforce initiatives for primary, community and social care. During 2021-22, these innovative models have been shortlisted and won national awards, including from HSJ and Nursing Times.
Barnet Federated GPs CIC is a not for profit organisation, putting GPs and healthcare professionals at the forefront of Primary Healthcare provision in Barnet.The Federation was formed in 2015 and aims to improve the quality of health in Barnet through sustainable primary care. Its current membership consists of all 50 primary care practices within the borough. Although we are a private company, we hold a strong affiliation to the NHS, its constitution and guiding principles.
