Job summary
We are looking for
Clinical Pharmacists to join one of the primary care networks (PCNs) based in
the North of Barnet working across 9 general practices.
As a Clinical
Pharmacist, you will have an undergraduate degree in pharmacy and registration
with the General Pharmaceutical Council. You will need to be self-motivated and
able to work on their own, have excellent written and verbal communication
skills, experience of influencing others and conducting clinical medication
reviews.
Experience in General Practice would be an advantage
but is not essential if the applicant is clinically up-to-date and understands
the process of a full medication review in the primary care setting.

Main duties of the job
This will be a
patient facing role working across a number of general practices for one of the
PCNs in Barnet, managing long-term conditions; working directly with patients
to assess and treat conditions, as well as promote self-care.
As a clinical
pharmacist, you will work within your clinical competencies as part of a
multi-disciplinary team to provide expertise in clinical medicines management,
provide face to face structured medication reviews, manage long term
conditions, management of medicines on transfer of care and systems for safer
prescribing, manage repeat prescription authorisations and reauthorisation as
well as acute prescription requests, while addressing both the public health
and social care needs of patients in the GP practice(s) that make up the PCN.

Barnet Federated
GPs CIC is a not for profit organisation, putting GPs and healthcare
professionals at the forefront of Primary Healthcare provision in Barnet. The
Federation is building a large clinical team who will work as one in developing
Primary Care Services in Barnet and will support Primary Care Networks.
The Federation was
formed in 2015 and aims to improve the quality of health in Barnet through
sustainable primary care. Its current membership consists of all 48 primary
care practices within the borough. Although we are a private company, we hold a
strong affiliation to the NHS, its constitution and guiding principles.

Essential
