Job summary

Job Title: Integrated Neighbourhood Team (INT) GP

3 individual GP roles, each operating across a neighbourhood - East / Central / West Haringey.

Hours of Work: 4-6 sessions per week of 4hr 10 mins per session

Contract Length: Fixed term 12 months (with a view to extension)

Remuneration:£12k per session (£108k WTE)

Responsible to:Associate Medical Director, Haringey GP Federation

Accountable to:Haringey Test-bed SROs (Chief Executive Officer Haringey GP Federation & Clinical Director, Adult Community Services Whittington NHS Trust)

Responsible for: Programme management of Haringeys Neighbourhood Test-bed

Location: Haringey Neighbourhood based (East, Central, West) with cross-borough working as required and occasional home visits. Flexibility to work remotely where appropriate

Main duties of the job

This is an exciting opportunity to take a leading role in supporting the implementation of neighbourhood health in the vibrant and diverse London Borough of Haringey. This role will form part of a joint team, hosted across GP Federation and Whittington Health NHS Trust. Both organisations have a strong and established history of working together to improve health outcomes for Haringeys diverse communities. Serving some of the most deprived areas in the country, we draw on complementary strengths, deep local insight and a shared commitment to delivering high quality, equitable care.

As two of Haringeys integrators , we work in partnership with the London Borough of Haringey to lead the development and delivery of the boroughs neighbourhood health model, recognised for its collaborative and innovative approach. As integrators, we act as enabling organisations responsible for supporting and scaling integrated, place based delivery of neighbourhood services. This role is central to the national ambition to shift care from acute settings into the community, with a strong emphasis on prevention, early intervention and reducing long term inequalities.

1.To provide clinical input at new weekly Complex Proactive MDTs (frailty and complex LTC) within the INT model

2.To provide clinical input to the MACC neighbourhood teams

Experience

  • Knowledge of NHS 10-year plan, Neighbourhood Health Frameworks and policy around hospital to community
  • Strong experience managing patients with frailty and complex long-term health conditions, including medications management.

Desirable

  • Experience of integrated care / anticipatory /proactive care models of working
  • Quality improvement and service development
  • Health inequalities and inclusion health
