Main duties of the job

This role is a job share with half of the role based with Haringey GP Federation and half of the role with Haringey North Central Primary Care Network (NC PCN).

The post holder will work with clinical and operational teams within a Primary Care Network (PCN) and across the GP Federation to create a vision to guide service improvements, identify opportunities to make tangible positive transformation, and provide the inspiration, resource and tools to deliver changes in a collaborative way. They will work alongside the wider leadership team including a digital clinical lead, quality improvement lead and clinical systems manager to ensure digital innovations best meet local needs.

The post holder will plan, coordinate and directly support delivery of transformation activity in general practice with a view to improving patient access, staff and patient satisfaction, and the efficiency and sustainability of general practice services.

The post holder will work closely with staff working at practice, PCN & GP Federation level to understand current service models, identify challenges, agree priorities for improvement and then support these to be delivered. The post holder will develop strong relationships with PCN member practices and will support the planning and delivery of improvements that will provide benefits to patients and staff.

PCNs build on existing primary care service sand enable greater provision of proactive, personalised, coordinated and more integrated health and social care for people close to home.

We work together to share and deliver local services to meet the needs of our population including vaccination hubs, social prescribing, proactive child weight management and more.

Essential

  • Demonstrable experience deploying digital innovations in a clinical care setting.
  • Demonstrable experience of effectively engaging with key stakeholders, such as clinical staff, system partners including Health Watch.
  • Demonstrable experience in project management to include planning, implementing, and managing projects across multiple organisations or teams.
  • Demonstrable experience of involving service users and carers in the design and delivery of health services.
  • Experience of working under pressure to tight deadlines.
  • Experience of working autonomously on a pre-determined piece of work.

Desirable

  • Demonstrable experience of producing accurate plans and project documents which inform and influence target audience.
  • Has previously led digital transformation across a broad and varied geography being able to support a single model where necessary but also support localism where also needed.
  • Experience of developing project plans and overseeing the delivery of projects to complete and finish.
