Community Wellbeing Advocate (Long-Term Conditions)

2.5 days a week

Main duties of the job

The Wellbeing Advocate role will focus on the second element. As a Wellbeing Advocate you will proactively work with the Turkish community in Haringey. This role will have two main responsibilities. First you will be meeting with Turkish patients one to one. Within these appointments you will be listening to patient concerns and providing advice, guidance, and support on improving these concerns and empowering patients to regain control over their lives and their conditions. This may involve signposting and referring to other services. Additionally, there is a community element. You will be reaching out to Turkish communities and groups, providing outreach with information about health and wellbeing.

Increase social contribution through Social Prescribing by encouraging clients to participate in wider community activities.

Signpost and refer patients to services in Haringey.

Support with benefits applications (Blue Badge, PIP, etc.) or signpost to council for help.

Improve access to wellbeing programs and services available in the wider community through the community advocate.

Work collaboratively with a cross-partnership response.

Seek to address the potential causes of barriers to healthcare in certain ethnic groups.

Work with VCSOs to create community engagement events creating awareness and engagement within targeted cohort.

Educate communities about long term conditions, health promotion, support on social issues. Consider inviting guest speakers.

Aim for regular monthly community events.

Co-design engagement events with specific themes and outcomes. Can be as creative as you want!

To develop events and social activities that encourage wellbeing e.g. cooking class, exercise group.

Participate and partnership with other Haringey charities and community speakers proactively find contacts to work with.

  • GCSE in Maths and English
  • Education up to A-Level

Desirable

  • Further education, degree education, or equivalent

Experience

  • Knowledge of Microsoft products
  • Knowledge of the importance of confidentiality and GDPR
  • Understanding importance of risk reduction and escalating issues once they occur
  • Understanding of how inequality and deprivation can impact health and wellbeing
  • Understanding of Long-term condition issues as experienced by individuals
  • Knowledge of long-term conditions including Chronic Kidney Disease (CKD) and Hypertension

Desirable

  • Knowledge of respiratory conditions such as COPD
