Job responsibilities
Primary Responsibilities:
- To deliver a high standard of patient care using advanced autonomous clinical skills with in-depth theoretical knowledge and evidence-based practice
- To manage a clinical caseload and seal with presenting patients need in a clinic setting
- To provided clinical leadership where appropriate within HHHIT and support other members of the team to develop and maintain clinical skills appropriate to your role and expertise
- Recognise the need for alternative working patterns and flexibility
Clinical Practice
- You will maintain clinical skills through appropriate continuing professional development (CPD) in line with annual appraisal
- You will work in line with all organisation policies and procedures, including human resources, safeguarding children, etc.
- Provide a point of contact within the health inclusion clinic (once an initial outreach review has been completed) for patients presenting with undifferentiated, undiagnosed problems, utilising history taking, physical examination, problem solving and clinical decision-making skills to establish a working diagnosis and management plan working in partnership with rough sleeping patients.
- Undertake clinic-based consultations for immediately necessary care or routine problems including detection and initial management of long-term conditions. In addition, undertake telephone calls and video consultations.
- Instigate necessary invasive and non-invasive diagnostic tests or investigations where there is capacity to do so or communicate the need for these to patients registered GP and interpret findings/reports within the scope of the GPs practices. Discuss the results and impactions of laboratory investigations with patients.
- Utilise clinical guidelines and promote evidence-based practice.
- Offer a holistic service to patients and their families (involvement of families is not routine, but possible), developing where appropriate an on-going plan of care/support with and emphasis on prevention and self-case.
- Refer patients directly to other services or agencies using appropriate referral pathways.
- Ensure safe handover of care within and outside the HHHIT service as appropriate.
- Work directly with members of the HHHIT and support integrated patient centred care through appropriate working with the wider primary care / social care networks / multiple agencies such as Mental Health, Substance misuse and local authority teams.
- Identify community health needs and participate in the development of patients/family-centred strategies to address them.
- Contribute to the HHHIT service quality targets to consistently achieve high standards of safe, evidence-based, cost-effective patient care and service delivery.
Professional Responsibilities
- You will maintain your professional registration, always working within the GPC code, and undertaking Revalidation and appraisal as required.
- You will promote and provide evidence-based care that complies with the relevant legal and policy frameworks
- You will work collaboratively with multidisciplinary teams across a range of care settings.
- Maintaining your professional registration working with the latest code of professional conduit (CIPD)
- Undertake statutory and mandatory training as required by the HHHIT service
- Demonstrate clinical leadership
- Pro-actively promote the role of the GP within the HHHIT service and externally to key stakeholders and agencies
- Respect patient confidentiality at all times and not divulge patient information unless sanctioned by the requirements of the role
Training and Development
- Participate in continuing professional development opportunities to keep up to date with evidence-based knowledge and competence in all aspects of the role to meet clinical governance guidelines for continuing professional development (CPD) and a personal development plan (PDP) utilising a reflective approach to practice.
- Undertake a variety of research and analysis tasks associated with the improvement of clinical care, medical diagnosis and treatment where appropriate using the following means.
- Audit of clinical practice
- Significant event review / root cause analysis
- Review of relevant literature
- Research unusual symptoms and treatments options through consultation with general practitioners, physicians and other specialists
- Participate proactively in a learning culture within the HHHIT
- Contribute to regular multi-disciplinary meetings as well as HHHIT and GP Practice educational meetings.
- Participate in multi-disciplinary protocol and patient group directions (PGDs) development as appropriate
- Work closely with other clinical staff and administrative manager in setting up and/or improving of the HHHIT systems for monitoring/measuring performance against clinical governance and quality indicator targets.
Administration
- Fully document all aspects of patients care and completed all required paperwork for legal and administrative purposes in accordance with relevant standards.
- Work in accordance with internal administrative systems relating to but not limited to the management of clinical data.
- Send and receive written information of behalf of the HHHIT relating to the physical and social welfare of patients.
- Ensure that all HHHIT service policies are fully implemented
Confidentiality / Data Protection / Freedom of Information
The post holder will have access to confidential information relating to patients, their carers, practice staff and other healthcare workers. They may also have access to information relating to the Federation as a business organisation.
All such information from any source is to be regarded as strictly confidential. Post holders must maintain the confidentiality of information about patients, staff, and other health service business in accordance with the Data Protection Act 2018. Post holders must not, without prior permission, disclose any information regarding patients or staff. Moreover, The Data Protection Act 2018 also renders an individual liable for prosecution in the event of unauthorised disclosure of information.

Infection Control is everyones responsibility. All staff, both clinical and non-clinical, are required to adhere to the Code of Practice on the prevention and control of infections and related guidance (2015) and make every effort to maintain high standards of infection control at all times thereby reducing the burden of Healthcare Associated Infections and complying with Government Directives, including COVID-19 regulations.

Within their sphere of competence, each member of staff is responsible for promoting and safeguarding the welfare of the children, young people and / or vulnerable adults for whom they are responsible or may come into contact with, in the job role.
