Job summary
This is an exciting opportunity to support your local Training Hub team in providing training and practical support to local GP practices to prepare for the new Long-Term Conditions (LTC) Locally Commissioned Service (LCS). The Training Hub has been commissioned to train and support the primary care workforce to deliver the LCS which will support patients to proactively manage their long-term conditions.
The post holder will be responsible for championing the model of care and providing training & and clinical guidance to GP practices within the borough for the new LTC LCS. This will include co-ordinating condition specific training requirements, Personalised Care & Support Planning & a wellbeing tool (MyCaW).
The role will work closely at an NCL level with a Programme Manager and Senior Project Officer, with 5 borough-based teams hosted in each borough of Business Change, Quality Improvement & Clinical Training Facilitators. Please refer to accompanying document for an organogram.

Responsibilities
1. LTC Specific responsibilities
- Quality Assurance of Training Resources against Practice Readiness Checklist - Provide clinical quality assurance of training materials provided by wider Training Hub team
- Personalised Care & Support Planning In partnership with Programme Manager, manage working relationship with accredited Personalised Care Support Planning Training Provider ensuring the training responds to priorities from frontline GP practices, integrates with the model of Care and has good uptake
- Condition specific training Co-ordinate mapping of condition specific training needs with support from Borough Clinical Facilitators and manage interface with ICB, UCL Partners and other external Training Providers to ensure training needs necessary for delivery of the LTC LCS specification are appropriately met
- Community of practice Work with the Training Hub team to continue to develop a community of practice for Long Term Conditions related work, including active signposting into other networks and Quality Improvement Methodology
- Targeted support Work with borough-based Training Hub team to identify and respond to queries from practices that require additional support to implement LTC LCS
2. Policy & Service Development
- Work with boroughs and providers to understand requirements and make recommendations to tailor the service which would enable localised and fit for purpose service delivery
- Make recommendations for changes and improvements to service delivery as appropriate
- Maintain a good knowledge of emerging policies from NHS, particularly regarding workforce development
3. System connectivity
- Ensure educational synergies identified and capitalised upon, including prevention of duplication of activities
- Support development of best practice for educators across NCL
- Through faculty steering group develop an NCL educator provider network HEE, HEIs, further education institutions to ensure information sharing and co-creating educator development/support.
4. Key Working Relationships
- Work with a broad range of internal and external stakeholders, including:
- North Central London Training Hub leads (across all boroughs)
- ICB Primary Care teams
- Place (i.e. borough) Partnerships (including any LTC Steering Groups or Age Well Boards)
- Primary Care Networks (PCNs) & General Practice
- Health Education England
- Local Medical Committee (LMC)
- Develop and maintain excellent relationships with providers across care settings, education providers and other partners to ensure continuous development of new career pathways, development, and implementation of new ways of working and relevant recruitment and retention strategies
- Develop and maintain excellent relationships with practices and local Higher Education Institutes
- Participate in relevant internal and external working groups/projects, services, and initiatives which may be highly complex, sensitive, political, and contain contentious information with the aim of providing information and analytical advice to the teams
5. Team Support and Supervision
- Provide supervision and support to Borough Clinical Facilitators for the LTC LCS programme in conjunction with the individual NCL Training Hub Borough Clinical Leads
- Support, motivate, and develop staff within the wider team to ensure that they can deliver the objectives for the programme and for NCL Training Hub
