Are you looking for an
exciting role providing medical oversight for community multi-disciplinary
teams working to prevent unnecessary hospital admissions for people whose
needs are best met in their own home?
We are looking for
regular experienced GPs to join our integrated team working flexibly on a
Monday and/or Tuesday. The hours
available include 08:00-20:00 or 08:00-14:00 and 14:00 20:00.
The role will be based
at Nelson House in Hereford.
This role would
ideally suit GPs who would welcome the time to unpick complex medical
histories and support proactive care planning; those with an interest in
frailty; and those who enjoy supporting the learning and development of
advanced practice colleagues.
Salary :- In hours rate from £46.12ph - 47.75ph,Otr of Hours rate ( 6pm-8pm & weekends) £55.34ph - £57.65ph.

Main duties of the job
The GP will provide
telephone and video support to the community ACPs, nurses and therapists
assessing patients in the community.
The GP will support
patients who are managed in a GP-run virtual ward. The GP will also support
paramedics and the ambulance service for patients who do not need conveyance
to hospital but may still need some medical intervention.
