WBC is a
small network of 3 closely located GP practices in Hereford City. WBC is high
performing with an established PCN Pharmacy team and motivated staff. We are
seeking an experienced Pharmacist to lead our team.
We are looking for a candidate who
iskeen to develop, manage and mentor
a team of pharmacists and pharmacy technicians
motivated to work as part of the
PCN Leadership Team
interested in supporting the networks
population health priorities, requirements of the PCN Directed Enhanced
Service and emerging Neighbourhood Health agenda.

support from experienced Pharmacists within the PCN and countywide
established working relationships
with general practice teams who are forward looking

Main duties of the job
As a member of the
multi-disciplinary team, you will be expected to provide prescribing advice and
support to practice clinicians, staff, and patients. This includes prescription
and medication queries, medicines policies and procedures, QOF and local
incentive schemes, audit, and medicines safety.
Clinical work at the individual practices will include
medication reviews, minor illness consultations and reviews post discharge.
You will have individual supervision
from a GP and PCN Prescribing Lead, and be part
of an established team of Pharmacists and Pharmacy Technicians. In Herefordshire we take a collaborative
approach across system partners e.g. other practices, PCNs, care homes and
the Acute Community Trust.To

city - based practices; Wargrave House Surgery, Belmont Medical Centre and

The post holder will be an experienced pharmacist, who acts within
their professional boundaries, to develop, manage and mentor a team of
pharmacists and pharmacy technicians to include regular clinical supervision, 1 to 1
support and development. They will be a
non-medical prescriber and have completed the Centre for Pharmacy Postgraduate
Education *CPPE* National Pathways and supervisor courses.
They will
work within their
clinical competencies as part of a multi-disciplinary
team to provide senior expertise and clinical leadership in medicines optimisation and
quality improvement across the PCN. This will

- Demonstrates the ability to communicate complex and sensitive information in an understandable form to a variety of audiences
- In depth therapeutic and clinical knowledge and understanding of the principles of evidence-based healthcare and strategies for improving prescribing in primary care, particularly for long term conditions
- Demonstrate good IT skills, with experience of spreadsheets, databases and reporting functionality
- Able to influence/motivate/persuade others with an empathic / reassuring manner, where there may be significant barriers
- Demonstrates a clear understanding of best practice and legal requirements relating to prescribing and medicines storage
- Ability to manage a team and provide individual clinical support and mentorship

- Understanding of the wider determinants of health, including social, economic and environmental factors and their impact on communities
- Able to identify and resolve risk management issues according to policy/protocol
- Experience of partnership/collaborative working and of building
