This is a fixed term maternity leave cover role.
We are excited to recruit to the post of Safeguarding Care Coordinator
within the East Primary Care Network *PCN*.
The post holder will be primarily working from home. There will be occasions when working from GP
practices and community settings is needed.
Mileage is paid at 45p per mile for additional travel that exceeds the
distance from home address to the base practice.

Main duties of the job
The Safeguarding Care Coordinator will have a key role in supporting Safeguarding Lead GPs to protect our most vulnerable patients. They will be an essential team member working with a Health & Wellbeing Coach, Social Prescribers, Occupational Therapists, Safeguarding Team and Primary Care Network.
The role will:
- support the PCN Safeguarding service delivery and provide co-ordination for safeguarding information and communication to support care planning.
- support the core responsibility of maintaining consistent protocols regarding safeguarding information across the network.
- support day-to-day safeguarding administration, document handling, record management and recording communications with partner organisations.
