This job is part time for a minimum of 16 hours per week.

Main duties of the job
The purpose of this role is to deliver nursing care to our patient population working with the nursing and wider clinical team at Fownhope Medical Centre.
Main responsibilities include:
- Working as a member of the practice multidisciplinary team, the post holder will ensure nursing services are delivered effectively to the entitled patient population.
- Assessing, diagnosing and treating specific diseases in accordance with agreed medical/nursing protocols.
- Undertake diagnostic, health screening, health surveillance and theraputic interventions within a broader health promotion strategy.

Job Summary
The post holder is responsible for the delivery of general practice nursing to the whole practice population. The focus of the role is the assessment of patient needs and the delivery of evidence-based practice for patients.
As an autonomous practitioner, the post holder is responsible for the care delivered, demonstrating critical thinking and skills in clinical decision making in the management of patients. The post holder will work collaboratively with the practice team in order to meet the needs of patients, support the development and delivery of policies and procedures.
MAIN RESPONSIBILITIES
Clinical Nursing Practice
- Assess, plan, provide and evaluate nursing care to meet the care needs of the individuals and groups in the practice population
- Assess, diagnose and treat specific diseases in accordance with agreed medical/nursing protocols
- Undertake diagnostic, health screening, health surveillance and therapeutic interventions within a broader health promotion/public health strategy.
Wound management
- Perform wound care using aseptic technique
- Use Dopplers to assess the circulation supply to wounds
- Undertake complicated dressings including leg ulcers
- Undertake suture and clip removal
- Assist doctors during minor surgery sessions.
Screening
- Undertake opportunistic screening including BP checks, height, weight, BMI, urinalysis, smoking status
- Carry out New Patient and over-75 checks
- Undertake opportunistic venepuncture.
Chronic disease management & health promotion
- Understand the annual monitoring requirements for a range of chronic diseases (such as diabetes, COPD, asthma, etc.) and undertake regular reviews of patients in line with best practice and targets as laid out in the Qualities and Outcomes Framework
- Recognise patient readiness to change and support their attempts to modify unhealthy behaviour
- Provide healthy eating advice to individuals or to groups including the underweight
- Provide smoking cessation advice individually in opportunistic consultations
- Provide individual advice to patients about increasing activity or exercise
- Offer advice on recommended safe limits for alcohol consumption
- Use INR star to monitor warfarin patients
- Develop and maintain the Health Promotion material within the practice and the waiting room notice board as well as on the practice website
- Refer as appropriate to external agencies, such as weight management groups.
Vaccinations & immunisations
- Administer injections under an individualised prescription or Patient Group Directive
- Ensure safe storage, rotation and disposal of vaccines and drugs.
Quality management & infection control
- Support patients and colleagues in managing quality and in adopting sound infection control measures
- Apply and record infection control measures within the practice according to local national guidelines. This includes:
- universal hygiene precautions
- the collection and handling of laboratory specimens
- segregation and disposal of waste materials
- decontamination of instruments and clinical equipment
- reporting and treatment of sharps injuries
- dealing with blood and body fluid spillages
- educate colleagues in adopting sound infection control measures.
Emergency management
- Competent in anaphylaxis and resuscitation techniques
Safeguarding
- Recognise the signs, symptoms and categories of child and adult abuse
- Understand the importance of recognition, observation, documentation and communication
- Aware of child health procedures, statutory local procedures and points of referral
- Has knowledge of the role of other practitioners and agencies

- Recognise and be aware of appropriate referral mechanisms for family violence, vulnerable adults, substance abuse and addictive behaviours.

- Support patients attending for emergency contraception and refer patients as appropriate
- Perform interval health checks for patients using hormonal contraception
- Advise patients on reducing risk of sexually transmitted diseases and refer patients appropriately.
Women's health
- Advise patients on the national and local Cervical Cytology programme
- Undertake a programme of cervical cytology sampling
- Audit minimum of twenty consecutive smears each year to demonstrate adequate sampling rates
- Promote breast self-awareness, demonstrate breast self-examination and provide supporting literature
- Advise patients about reducing the risk of sexually transmitted diseases and make appropriate referrals
