Job summary

We have an exciting opportunity for an Advanced Care Practitioner in the North and West Herefordshire Primary Care Network.

The successful candidate will work as an autonomous practitioner working independently in General Practice to provide care for the presenting adult or child from initial history taking, clinical assessment, diagnosis, treatment and evaluation of care. They will demonstrate safe, competent clinical decision-making and expert care, including assessment and diagnostic skills, for all patients within the general practice. The post holder will apply critical thinking and demonstrate safe clinical decision-making. They will communicate and work collaboratively with the general practice team to meet the needs of patients, supporting the delivery of policy and procedures, and providing clinical leadership as required.

The post holder must possess competencies and capabilities across all four pillars of advanced practice.

We are a highly organised and forward looking PCN with strong relationships between practices and community providers. We can provide you with personal and professional development and a supportive friendly team to work with. You will be allocated a GP with whom you will have access to regular clinical supervision.

Main duties of the job

The successful candidate will be part of the teams of two Practices within the N&W PCN.

  • Practise in compliance with their respective code of professional conduct and within their scope of practice, being responsible and accountable for their decisions, actions and omissions at this level of practice.
  • Demonstrate a critical understanding of their broadened level of responsibility and autonomy and the limits of own competence and professional scope of practice, including when working with complexity, risk, uncertainty and incomplete information.
  • Undertake safe clinical decision-making and expert care, including assessment, diagnostic skills and safety netting for adults and children using face to face (clinic and home visits), telephony and on line consultation platforms.

  • Work in partnership with individuals, families and carers, using a range of assessment methods as appropriate (e.g. history-taking; holistic assessment; identifying risk factors; mental health assessments; requesting, undertaking and/or interpreting diagnostic tests; and conducting health needs assessments).

The North & West Herefordshire PCN includes five General Practice, each of which offers high quality patient care delivered by a supportive team. You will work as part of a wider PCN team with Pharmacists, Paramedics and a Wellbeing Team comprising of Social Prescribers, Health & Wellbeing Coaches, Care Coordinators, an OT and Dietitian all responding to the wide-ranging health needs of the population.

  • Exercise professional judgement to manage risk appropriately, especially where there may be complex and unpredictable events and supporting teams to do likewise to ensure safety of individuals, families and carers.
  • Work collaboratively with an appropriate range of multi-agency and inter-professional resources, developing, maintaining and evaluating links to manage risk and issues across organisations and settings.
  • Act as a clinical role model/advocate for developing and delivering care that is responsive to changing requirements, informed by an understanding of local population health needs, agencies and networks.
