The S&W
PCN are delighted to be recruiting for a Health & Wellbeing Team Lead.
The Health & Wellbeing
Team Lead will have management oversight for the PCN wellbeing team. The current team includes a Care
Coordinator, Social Prescribers and a Health & Wellbeing Coach. As the team expands the staff may include a
Mental Health Link Worker and other roles from the Additional Roles
Reimbursement Scheme (ARRS), who will all work collectively to support the
wellbeing of patients in our network.
As a management position
within the PCN, the position will also lead on health inequality strategy for
the network and on building community
partnership initiatives within the locality.
As an experienced practitioner they will also be expected to influence
and guide improvement at a county level.

Main duties of the job
This
is an exciting opportunity to take ownership for the health and wellbeing
strategy within the SW PCN. Working
with GP practices, NHS colleagues, social care and the voluntary sector, the
role will identify the specific needs of
our patient population and proactively design and manage our approach to supporting
their health and wellbeing.
The
main responsibilities of the role will be as follows:
Day-to-day line management of the
Health and Wellbeing team within the Primary Care Network (PCN). To include, recruitment, training,
supervision, case management support and appraisal.
To develop and manage the referral
pathways within the team, and also into and out of the health and wellbeing
team from a range of partners
To
lead on the development and implementation of strategies to mitigate Health
Inequalities in the locality
Where
appropriate, to manage a small caseload of patients as appropriate to the
post-holders skill set
About us
The
S&W
PCN GP
practices serve the market town of Ross-on-Wye and surrounding rural communities,
responding to the wide-ranging health needs of the population. The network is
made up of Alton Street Surgery, Pendeen Surgery, Fownhope Surgery, Golden
Valley Practice, Much Birch Surgery and The Surgery Kingstone. While there will
be flexibility to include some remote working, the role will be expected to
maintain a presence in Ross-on-Wye and work on location at surgeries and in the
community as required.

emergency services, legal and welfare advice services, VCSE organisations, and

care and support roles.
To participate in the PCN leadership and Board meetings.
To lead on the development and implementation of strategies to mitigate
Health Inequalities in the locality.
To partner with colleagues across Herefordshire General Practice
on collective initiatives that will support the wider health and wellbeing
community. For example, developing key
performance indicators, competency frameworks and appraisal structures for roles
within the teams. Or designing health
inequality strategies that can be implemented across the county. (Examples not exhaustive).
