Care Coordinator - Data Analysis, North and West PCN

Location: GP practices based in North and West Herefordshire PCN with some remote working

The North & West Herefordshire PCN has recognised that high quality data input and analysis is pivotal to our ability to identify segments of our patient base to focus on. We are delighted to recruit a care coordinator to support the practices in managing the data integrity of care recording so that high-quality care planning can be done effectively.

The care coordinator will lead on local PCN data integrity initiatives, but as an experienced practitioner they will also work with the wider business intelligence team for the County.

Main duties of the job

This is an exciting opportunity to work with GP practices to identify the specific needs of their patient population and match support opportunities to those needs.

Main duties of the role are as follows:

To facilitate the effective delivery of patient-centred, personalised health and social care plans for patients by ensuring that all EMIS templates in GP surgeries are set up to capture the most up-to-date clinical codes

To work closely with the ICS Primary Care Analytics Team to form part of a wider General Practice Analytical Network to ensure that reporting of activity is done accurately

To work with clinical leads to establish and run multi-disciplinary team meetings and ensure that follow up occurs

To ensure that information about the impact of patient care interventions is captured and reported so that the PCN can adapt its strategy to ensure high-value, evidence-based care is provided

To provide updates to the PCN board about progress against contractual goals and monitor trajectory across the year

The North & West Herefordshire PCN GP practices serve rural and market town communities responding to the wide - ranging health needs of the population. The network includes Kington Medical Practice, Ryelands Surgery Leominster, The Mortimer Medical Practice Kingsland, Tenbury Wells Surgery, and Weobley and Staunton on Wye surgeries. It is a highly organised and forward looking PCN with strong relationships between practices and community providers.

There will be an opportunity to work remotely.

The Care Coordinator will be working with established general practice teams who have longstanding relationships with their local community and are committed to providing lifelong care in this beautiful part of Herefordshire.
