The post holder will help support the provision of continuity of care for patients at Much Birch Surgery, as they access services at the surgery and across the wider Primary Care Network. The ideal candidate will relish the opportunity to work within a fast paced and often challenging environment. They will be responsible for the call and recall process for management of long term conditions and for identifying patient groups who would benefit from proactive health support.
This is a full time position however, a job share could be considered.
Interviews will be held on Wednesday 31st August 2022.
Main duties of the job
The
purpose of this role is to support the practice administration team in the
provision of population health care.
Main duties will include:
·
Running reports from our clinical software system (EMIS)
to analyse groups of patients to target for health interventions
·
Dealing with patient enquiries
·
Monitoring of admin emails
·
Co-ordination and processing of lab reports and
other data from external documents to patient records, adhering to data
protection legislation
·
Help the practice to hit its contractural targets
in the Quality and Outcomes Framework (QOF) and all local and national
enhanced services
·
Process GP2GP, deductions and registrations
·
Support Practice Manager and Reception Manager
with organisation of clinical staffing rota

Job responsibilities
The post holder will help support the provision of

services at the surgery and across the wider Primary Care Network. They will be responsible for managing the
call and recall process for chronic disease management and for identifying
patient groups who would benefit from proactive health support. The ideal candidate will have experience
working in Primary Care and possess excellent communication and organisation
skills, with a demonstrable passion for enhancing patient experience and
outcomes.
Duties and
responsibilities:
· Utilise
population health intelligence to proactively identify and work with a cohort
of patients to deliver personalised care.

Capture key information to enable
comprehensive and accurate records of support and input these into clinical
systems adhering to data protection legislation.
·
Ensure that patient medical records are kept
up to date with the latest communications from third party health care
providers.
·
Support the practice to hit its contractual
targets in the Quality and Outcomes Framework (QOF) and all local and national
enhanced services.
·
Work with the administrative team to ensure
the accurate and timely completion of patient workflow and referrals.
