Job summary

We are excited to recruit to the post of Health and Wellbeing Coach, to work within our Primary Care Network providing 1:1 personalised support to people who are referred to our multi-disciplinary Wellbeing Team.

The successful candidate will have good communication, negotiation and people management skills and act with compassion and integrity.

They will have experience of using coaching approaches, frameworks and models or other helping strategies, for example, motivational interviewing.

This is a vital role within the Primary Care Network and the post holder will provide health and wellbeing coaching for the most vulnerable people in the community, including the frail / elderly and those with other long-term conditions.

They will work closely with GP and practice teams to manage a caseload, ensuring appropriate support is available to people, families and carers. The successful candidate will work alongside social prescribing link workers and care coordinators to provide an all-encompassing approach to personalised care and enable people to work out how best to use the health and care system.

Main duties of the job

This is a developing role. You will work with the multi-disciplinary team to understand the population health of our network and ensure the role delivers the best possible outcomes for our patients. Duties will include:

Using coaching skills to support people with lower levels of patient activation to develop the knowledge, skills, and confidence to manage their health and wellbeing, whilst increasing their ability to access and utilise appropriate services.

Delivering group, video group and 1 to 1 consultations to meet personalised care plans; engaging with patients to help them take an active role in their health; providing advice and support for both patients and colleagues.

Supporting practice colleagues with health promotion initiatives including prevention and management of long-term conditions.

Working in partnership with other health, social care and voluntary sector providers to support and develop local services.

  • Educated to GCSE or equivalent.
  • Demonstrable commitment to professional and personal development
  • NHSE Approved training in motivational coaching, behavioural change, and goal setting.
  • NB: This is a 4-day course is due to be released by NHSE shortly. Completion of the training will be an essential requirement for new Health & Wellbeing Coaches.

Desirable

  • NVQ 3 or equivalent and/or relevant basic/first level professional qualification or working towards this.

Experience

Essential

  • Experience in health promotion and health improvement.
  • Experience of data collection and using tools to measure the impact of services e.g., Patient Activation Measure.

Desirable

  • Experience of working directly in a community development context, adult health and social care, learning support or public health/health improvement (can include unpaid work).
  • Experience of supporting people, their families, and carers to change health behaviours and improve wellbeing (can include unpaid work)
