We have an exciting opportunity
for a non-clinical care coordinator to join our Health & Wellbeing Team
in North & West Primary Care Network. The successful candidate will play
a key role in supporting the Clinical Safeguarding Care Coordinator and work
alongside the existing Care Home Care Coordinator to protect our most
vulnerable patients.
The
post holder will be a key member of the developing Wellbeing Team working
closely with Care Coordinators, Social Prescribers, Health & Wellbeing
Coaches, a Mental Health Practitioner, Occupational Therapist and Dietician.
Main duties of the job
You will work within N&W PCN
GP Practices to support central co-ordination for safeguarding children and
adults information, communication and patient care planning for adults living
in residential / nursing care home settings. Working within the care home
environments will provide with an opportunity to coordinate any safeguarding
concerns too, as required.
You will support
multi-disciplinary team working and provide assistance with liaison between
the Primary Care Network team, acute, voluntary and community services, Multi
Agency Safeguarding Hub and the wider Integrated Care System.
You will support with safeguarding
administration, document handling, record management and communication with
partner organisations. To also support continuity of care, facilitating smooth
and planned discharge and handover between care settings across the health and
social care system.

This is a full time position working 37.5 hours per week.
