We are looking for an innovative, motivated Care Coordinator
to join East Herefordshire Primary Care Network (PCN) to
improve Cancer & Safeguarding processes across the
network. You will work with the practices as part of our multi-disciplinary team to understand the population health of our
network and ensure the role delivers the best possible
outcomes for our patients.
This is an exciting opportunity to work closely with our GP
practice teams to coordinate services for our local population,
helping to make a real difference to people's lives.
Main duties of the job
This is a new and developing role. You will work in
partnership with clinical and non-clinical colleagues to ensure
the role delivers the best possible outcomes for our patients.
Duties will include:
Supporting practice colleagues to not only improve cancer
screening rates, but to deliver against safeguarding & cancer
quality targets which will in turn improve longer term
outcomes for our patients.
Working in partnership with other health, social care and
voluntary sector providers to support coordination & delivery
of local services..
Providing a central co-ordination role for patient care planning
and Cancer Care/Safeguarding review processes. This will
involve working with practice teams and wider partners to
ensure best practice around Cancer Care & Safeguarding.
Working as an essential team member within a multi-disciplinary team

The East Herefordshire PCN includes Ledbury Health
Partnership, Cradley, Colwall & Nunwell practices who
together serve almost 30,000 people. The network is very
fortunate to have 2 dynamic GP Clinical Directors and very
forward-thinking practice teams who are committed to
improving the health and wellbeing of their patients and
supporting their teams to develop in their roles.
Although the population in East PCN lives longer than the
national average there are higher than local average levels of
Frailty, Obesity, Depression & Alcoholism. In addition there is
a need to improve cancer screening rates amongst our
patients, & we hope that this role will be key to this
improvement.

JOB PURPOSE:
The Care
Coordinator will work with practices across the East Primary
Care Network (PCN) supporting and assisting practices with Cancer Care Review
and Safeguarding processes and delivery of other health initiatives.
The Care Coordinator will:
Work within our network of East Herefordshire GP Practices to provide
a central co-ordination role for patient care planning and Cancer Care/Safeguarding
review processes. This will involve working with practice teams and wider
partners to ensure best practice around Cancer Care & Safeguarding.
Provide administration support to practices to update patient
registers such as the palliative care register and arrange MDT meetings. The
Care Coordinator will also support practices with audits & contractual
requirements ensuring that all patients are appropriately identified and coded.
Work as an essential team member within a multi-disciplinary team.
Provide feedback to the practices, troubleshoot and escalate
actions as necessary, providing advocacy for service users.
Main Responsibilities
1.
To
build relationships with the practices & work with secretarial/ admin and
reception staff and external agencies e.g .Safeguarding, Cancer Care Teams to embed
best practice.
2.
To
review all new 2ww referrals within 24 hours to ensure that appropriate
investigations have been organised urgently by practice (eg. Bloods/ MSU) and
that results will be available before the 2ww clinic appointment.
3.
To
review each new 2ww referral/new Cancer diagnosis & compile report which includes
key information.
4.
To take part in structured cancer care reviews with a focus around
social/financial needs, and escalate to GP where there are any medical needs
5.
To
review/collate data as per contractual requirements
6.
To
work alongside the GP Cancer & Safeguarding practice leads as a point of
contact for any practice/ clinical questions and to help complete the audit of
new cancer diagnosis, particularly looking at areas which may have caused a
delay in diagnosis.
7. To organise regular network cancer/safeguarding meetings
with all members of the team to & prepare necessary reports/data.
8. To collate/present & submit data for
the National Cancer Diagnosis and other Safeguarding audits.
- To ensure
Safeguarding policies and processes are up to date and maintained at each
practice within the PCN
- To
maintain a register of vulnerable patients at each practice within the PCN
- To review
clinical records to prepare accurate and concise safeguarding reports and
chronologies
- To
organise and attend MDT meetings to review vulnerable patients, presenting
information as required and documenting actions required
- To
support practices with all external requests for Safeguarding related
information and reports
- To develop
effective working relationships with external agency Safeguarding contacts
and promote local referral pathways within the practices.
