General Practice is undergoing significant change with opportunities emerging to shape and improve service quality and safety.

Taurus Healthcare, Herefordshire’s GP Federation is supporting Primary Care Networks (PCNs) at an exciting time as they develop and grow to improve care and the health of populations they serve. This nursing and quality leadership role provides a key quality assurance role for Taurus services and supports local GP practices and PCNs to develop new ways of working and learning across General Practice and with partner organisations collaboratively across Herefordshire.

Main duties of the job

We are looking for an experienced professional who is passionate about high quality patient centred care, able to work dynamically in a changing and challenging environment with strong leadership skills and a can do attitude.

Job responsibilities

The role comprises of a number of key functions

· To support the Nursing and Quality Directorate to:-

Ø deliver the organisation’s quality, risk and regulatory compliance agenda to meet local, regional and national requirements

Ø deliver corporate quality and safety objectives

Ø deliver the organisation’s quality, risk and regulatory compliance agenda to meet local quality schedules, regional and national requirements

Ø ensure effective incident and complaints processes are in place with an embedded no blame learning and safety culture

Ø provide expert professional, safeguarding and infection control advice

Ø support the development of PCN quality infrastructure and learning

Ø Support practices with infection prevention control and CQC expertise

· To Chair clinical governance forums

· To lead and deliver the clinical workforce quality assurance agenda in alignment with professional and clinical role requirements including HEE road maps across Taurus services and support wider general practice

· Provide Safeguarding Lead function for Taurus Healthcare

· Develop Patient Safety Specialist Function

· Deputising for the Director of Nursing and Quality

· Participate in the senior management on call rota

Main Responsibilities

Quality, safety and risk management

· Promoting a just and safe culture across the organisation.

· Ensure quality assurance strategies and operational frameworks meet and delivery legislative and regulatory requirements and remains compliant

· Ensuring regulatory and contractual requirements are in place

Ø Support organisation risk management functions, ensuring provision of dynamic high quality reactive and proactive approaches focused on delivering high standards of patient and staff safety through:-

    • Incident reporting and management system
    • Investigations management
    • Complaints
    • Litigation

Ø Ensure risk, incident and complaint management activities are undertaken and owned within Taurus Committees and wider general practice (as at scale activities develop) supported by robust reporting structures and risk registers

· Ensure co-ordination of organisation governance activities, working towards improved clinical outcomes, patient safety and patient experience

· Develop and participate in health community wide governance networks and activities in order to enhance the whole patient pathway, and promote integration of partnership working

· Encourage the use and flow of appropriate information throughout the organisation, in order to provide evidence of lessons learnt, sharing of best practice and of continually improving practice

· Ensure follow up of all related internal and external governance and quality related reports, coordinating the development and implementation of action plans, and their delivery within agreed timescales

· Provide strategic oversight and operational expertise to ensure the Patient Safety software system (Datix) which records incidents, complaints, compliments and risk functions effectively for Taurus Healthcare and wider primary care (GP practices and PCN’s.)

· Provide quality and safety leadership, advice, training and education to GP practice and PCN colleagues

· Work in partnership with Integrated Care System colleagues and PCN’s to develop a quality framework and delivery mechanism to enable place based working

· Working in partnership with colleagues across the system to support quality and safety improvement of primary care services and develop ‘at scale’ and PCN solutions

Support quality and safety assurance functions which include: -

Ø Deputising for the Director of Nursing & Quality as needed

Ø Overseeing processes that ensure the highest standards of clinical governance are maintained within the organisation

Ø Ensuring risk management processes are correctly followed to identify, manage and appropriately escalate risks

Ø Ensuring effective mechanisms for patient feedback are in place supporting continuous learning and improvement plans

Ø Ensuring quality and safety reports meet Taurus Quality Assurance Committee (QAC) and Board requirements and contractual quality schedules including annual reports

Ø Monitoring, investigation and analysis of incidents, complaints and compliments extracting lessons learnt and coordinating action plans utilising recognised tools such as root cause analysis

Ø To support development of audit programmes

Ø Developing approaches to align with NHSE Patient Safety Incident Response Framework

Ø Represent Taurus at relevant quality/governance meetings and implementing agreed actions

Ø Overseeing the review process for policies, procedures and standard operating procedures

Ø Management of Quality and Safety Administration role

Clinical Workforce

Work with Training Hub, clinical service, PCN teams and wider general practice to embed a robust frameworks to support

· Education, competence and supervisory frameworks and training plans being in place and monitored, including HEE road maps across

· Development of professional audit plans and reporting into clinical governance forums

· Workforce planning activity aligned to HEE star methodology

· Reviewing of clinical role related policies, procedures, and SOPs for Taurus employed staff

Professional leadership role

· Provide highly visible leadership and foster a culture which values continuing professional development that empowers professionals to achieve excellence in the delivery of patient care

· Provide strategic and operational advice and support for federative and provider activities

· Support development of care pathways, focusing on person centred approach in co-production with patients

Supporting Senior Management Team (SMT)

· Provide support and professional advice to operational, clinical and executive colleagues

· Participate in the senior management on call rota
