An exciting opportunity has arisen for a Primary Care Network Development Manager to work for the South & West Primary Care Network, Herefordshire. This would suit a person with healthcare management background.

South & West Primary Care Network is a network of 6 practices – Alton Street, Fownhope, Golden Valley, Much Birch, Pendeen and Kingstone - with a list size of over 41,000 patients, and a dynamic GP Clinical Director supported by committed practice teams and continues to go from strength to strength.

The PCN Development Manager will form an integral part of the PCN leadership team working alongside the Clinical Director, PCN board members, Practice managers, partner organisations and communities themselves, to ensure the delivery of the Primary Care Network’s priorities.

Main duties of the job

The PCN Development Manager will support the PCN to develop and implement service improvement and transformation work aligned to local and national priorities. This includes building on current primary care provision and working proactively with the wider network to ensure we can offer our local population coordinated and more integrated health and care. The role entails building effective working relationships, working closely with the Clinical Directors, engaging stakeholders across different organisations and disciplines, coordinating work-streams, and collating and monitoring risks and issues.

The PCN Development Manager will act as point of expertise, coordinate the successful delivery of all contractual requirements, translate national and local policy into meaningful changes, and will support the network to achieve improved outcomes for their local population. They will also liaise with the PCN Development Managers of other PCNs both within Herefordshire and across the ICS.

GP practices retain their individual leadership and responsibilities but are supported by a wider leadership team underpinned by the federation; this enables us to partner with local health and social care organisations, to help coordinate prevention and early intervention and maximise opportunities for more ‘joined up’ care.

PRIMARY DUTIES AND AREAS OF RESPONSIBILITIES

Key Responsibilities and Duties:

The PCN Development Manager’s areas of responsibility include:

  • First point of contact for Practice Managers and partner organisations with regard to delivery of Network’s projects/schemes.
  • Manage and deliver contractual requirements on behalf of the Networks.
  • Co-ordinate training and development of PCN clinical/administrative staff.
  • Co-ordinate the delivery of PCN plans, including PCN development, service improvement plans, and plans associated with contracted activities.

· Support the recruitment, induction and training of new and existing staff groups

  • Collate feedback / analyse data on behalf of the Network to report to the CCG and other organisations as required.
  • To manage the workflow processes in relation to the above
  • To ensure an adequate record of meetings is taken and circulated on a timely basis, ensuring key agreements and actions are logged and monitored for completion.
