Fownhope Medical Centre is looking for an ANP or ECP to join the Duty Team of clinicians at our friendly rural surgery. As an independent prescriber this role will work alongside our current GPs to provide same day care to our patients.

Main duties of the job

The purpose of this role is to work as an autonomous clinical practitioner within the Duty Team. Main duties include:

- Assess, diagnose, plan, implement and evaluate treatment and interventions for patients presenting with an undifferentiated diagnosis

- Prescribe and review medication for therapeutic effectiveness

- Undertake home visits as required

- Support and share responsibilities for additional clinical workflow, such as documents, prescriptions and Lablinks

Job Summary

To perform as an autonomous clinical practitioner working independently and with other health care professionals in the Duty Team of the surgery to assess, diagnose and treat conditions of patients attending within a primary care setting, including the prescribing of medication.

Provide expert professional advice to patients, carers and colleagues and ensure the maintenance of clinical excellence.

To develop new and innovative concepts, models, methods and practices to deliver new and improved primary care services to meet the needs of the Practice population.

To be professionally accountable and responsible for all elements of clinical practice in accordance with the NMC code of conduct or other governing body.

Key Responsibilities

Clinical Practice

· Assess, diagnose, plan, implement and evaluate treatment/interventions and care for patients presenting with an undifferentiated diagnosis

· Clinically examine and assess patient needs from a physiological and psychological perspective, and plan clinical care accordingly

· Prescribe and review medication for therapeutic effectiveness, appropriate to patient needs and in accordance with evidence-based practice, national and practice protocols, and within scope of own practice

· Assess, diagnose, plan, implement and evaluate interventions/treatments for patients with complex needs

· Provide information and advice on preserved or over the counter medication on medication regimes, side effects and interactions

· Prioritise health problems and intervene appropriately to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency care

· To be professionally accountable and responsible for all elements of clinical practice in accordance with the NMC code of conduct or other governing body.
