Job summary

WBC Primary Care Network (PCN) is looking for an enthusiastic, full time Nurse Associate to join our growing team.

As a Nurse Associate in general practice, you will assist the GPs and Nursing teams to deliver safe, high quality primary care services to our patients.

WBC is a network of 3 GP practices, looking to improve proactive care to reduce health inequalities in Hereford City. This is an exciting opportunity to work with our Health Inequalities Lead and multi-disciplinary team including: Senior Social Prescriber, Health & Wellbeing Coach, Pharmacy team and Care Coordinators.

This Wellbeing Team will be focussing on supporting patients identified as having long term conditions, particularly those who are not currently engaging with their care.

We are seeking to expand the PCN team with more nursing support and look at innovative ways to deliver care including group consultations and outreach. The Nurse Associate will be a key player in this development.

Main duties of the job

Performing and recording clinical observations and promoting health and wellbeing as part of chronic disease management.

Supporting population health programmes including cervical screening, hypertension management and CVD prevention, vaccination campaigns and spirometry. (Training will be provided where necessary).

Advising patients on general healthcare and promoting self-management where appropriate, including signposting patients to personalised care colleagues and local community and voluntary sector services working in collaboration with the PCN Wellbeing Team.

Working with practice teams and PCN MDT to ensure delivery of nursing associate duties which complement existing workforce.

The WBC Nurse Associate will receive day to day support from registered nurses within each practice and mentoring from our Nursing Lead.

JOB PURPOSE:

The role of the Nursing Associate will be to work with the Registered Nurses and other members of our diverse clinical team, providing and monitoring care to patients within the network practices and in community settings. The Nursing Associate will be supported on a day-to-day basis by the clinical team at each GP surgery to deliver holistic care to our patients.

This is a new post with a specific focus on addressing Health Inequalities for the Primary Care Network. The Nursing Associate will be mentored by the Nurse Lead Coordinator for WBC, who is also the Health Inequalities Lead and a practising clinician.

The Nursing Associate will be a key member of the multi-disciplinary team supporting population health programmes including cervical screening, hypertension management and CVD prevention, vaccination campaigns and spirometry.

Training will be provided where necessary.

There are also plans to develop group consultations and community outreach through the Wellbeing Team which will provide an opportunity for the Nursing Associate to support population health management for the network.

We will encourage and support your continued professional development as well as assisting in developing a specialist area of interest where possible.

You will be practitioner who is comfortable working independently and under the leadership of our Registered Nurses, supporting with all aspects of nursing care and improving on the safety and quality of patient care at every opportunity. You will need excellent communication skills, be able to meet targets and liaise positively with patients, primary care staff and other partners.

To maintain a high-level professional standard of nursing care in adherence to all Practice protocols/policies & guidelines, as well as to provide nursing assistance to the GPs and other members of the primary healthcare team.

Perform and record clinical observations and promote health and wellbeing as part of chronic disease management. Escalate abnormal finding as appropriate in line with Practice policies.

To support and provide excellent nursing care.

Ability to deliver holistic care with an awareness of long-term conditions and other co-morbid physical and psychological presentations.

Advise patients on general healthcare and promote self-management where appropriate, including signposting patients to personalised care colleagues and local community and voluntary sector services working in collaboration with the PCN Wellbeing Team.

Maintain accurate and contemporaneous patient health records and enhance your own performance through continuous professional development, imparting own knowledge and behaviours to meet the needs of the service.

Recognition of the limitations of your own safe practice and when to ask for additional clinical support.

Excellent organisational skills and the ability to manage own caseload.
