An exciting
opportunity has arisen for a Primary Care Network Development Manager to work
for the East Primary Care Network, Herefordshire. This would suit a person
with healthcare management background.
East Primary Care Network is a network
of 4 practices - Colwall Surgery, Cradley
Surgery, Ledbury Health Partnership and Nunwell Surgery - with a list size of over 30,000
patients, and 2 dynamic GP Clinical Directors supported by committed practice teams
and continues to go from strength to strength.
The PCN Development Manager will form an
integral part of the PCN leadership team working alongside the Clinical
Directors, PCN board members, Practice managers, partner organisations and
communities themselves, to ensure the delivery of the Primary Care Networks
priorities.
Main duties of the job
The PCN Development
Manager will support the PCN to develop and implement service improvement and
transformation work aligned to local and national priorities. This includes
building on current primary care provision and working proactively with the
wider network to ensure we can offer our local population coordinated and
more integrated health and care. The role entails building effective working
relationships, working closely with the Clinical Directors, engaging
stakeholders across different organisations and disciplines, coordinating
work-streams, and collating and monitoring risks and issues.
The PCN Development
Manager will act as point of expertise, coordinate the successful delivery of
all contractual requirements, translate national and local policy into
meaningful changes, and will support the network to achieve improved outcomes
for their local population. They will also liaise with the PCN Development
Managers of other PCNs both within Herefordshire and across the ICS.

across 24 hours, 7 days a week.
GP practices retain their individual
leadership and responsibilities but are supported by a wider leadership team
underpinned by the federation; this enables us to partner with local health
and social care organisations, to help coordinate prevention and early
intervention and maximise opportunities for more joined up care.

At all times the post holder must act in a manner
consistent with the code of conduct representing Taurus Healthcare,
Herefordshire General Practice and the NHS.

another PCN Development Managers, employed for 37.5 Hours/ Week.

Herefordshire PCN
- Support the PCN
Clinical Directors and Board to ensure the smooth running of the Network
- Working closely with the PCNs Practice Managers and
other key stakeholders develop a culture of support and inclusivity
- To be involved in
the management and development of the Network, ensuring
the delivery of all PCN contractual requirements
- Responsible for
the delivery of PCN DES, including delivery of targets associated with the
PCN Investment & Impact Fund
- Responsible for
developing and delivering detailed workforce plans, aligned to PCN
priorities and reflecting opportunities within the Additional Roles
Reimbursement Scheme (ARRS)
- To ensure all PCN
staff, specifically new roles appointed via the Additional Roles Reimbursement
Scheme (ARRS) have clear objectives, development plans, and robust
management arrangements in place including clinical supervision where
needed and operational support for day-to-day service delivery. Initially,
this will require the direct line management of all PCN Additional roles.
As the team grows, it is anticipated that this responsibility will require
a dedicated operational manager, and it would make sense for this post to
report directly to the PCN Development Manager.
- To be responsible
for the production of performance and quality reports within the Network
and other Network initiatives
- To be responsible
for the oversight of the Networks financial systems, including payments
to all the practices within the Networks, working closely with the finance
team at Taurus Healthcare and the CCG.
- Ensure the PCNs
compliance with the GDPR and FOI along with other relevant legislation and
professional information management standards such as GMC/BMA and LMC guidance
- To represent the PCN
and its CDs in cross organisational meetings when agreed e.g. locality/CCG meetings/ICS and other H&W
wide meetings
- Working together
with the PCNs Practice Managers support delivery of PCN vaccination
programmes, ensuring delivery of NHSE targets. To take a lead for the PCN to
communicate with relevant stakeholders.
