Duties and Responsibilities

This is primarily a clinical post within a research project. The postholder will work closely with Primary Care Networks in North London to oversee the delivery of an exciting new primary care service delivery approach. Responsibilities include supporting and upskilling staff in the provision of dementia care, as well as managing a small complex caseload.This role sits within the PriDem research project, which aims to improve primary care led post-diagnostic dementia support.The PriDem research project office is based at the Centre for Ageing Population Studies; Research Department of Primary Care and Population Health, University College London.

The post is funded until the end of October 2022 in the first instance. We are open to secondments and job shares.

Duties & Responsibilities:

Responsibilities will relate to four key tasks, outlined below.

Key task 1 - Building capacity

Provide leadership, mentoring and training to primary care teams, to support person-centred dementia care

Work collaboratively with health & social services and with the voluntary and independent sectors (as appropriate) to support best practice in dementia care

Share up-to-date knowledge and good practice in dementia care

Key task 2 - Developing systems to enhance post diagnostic dementia support

Support involvement of key stakeholders, including people living with dementia and carers, in developing and improving dementia care systems

In partnership with primary care staff, identify and address barriers to delivering post diagnostic support

Key task 3 - Case management of complex people living with dementia & carers

Provide specialist assessment, evidenced based intervention, advocacy, advice and emotional support for people living with dementia and carers with complex needs

Maintain clinical records using relevant information systems and provide verbal and written feedback as appropriate, to people with dementia, carers, referring agencies and professional colleagues

Carry out risk assessments and management of the physical environment to safeguard the person living with dementia and carers to enable complex health needs to be managed within the appropriate environment

Contribute to strategies designed to improve access to services for people with dementia and carers who are socially excluded

Key task 4 - Supporting & contributing to the PriDem study

Contribute to the development and evaluation of the PriDem intervention through participation in meetings with the research team, interviews, observation of practice and by maintaining a reflective diary.

Collate and record data accurately and in a timely manner.

Additional general responsibilities:

Ensure care is delivered in accordance with relevant standards (e.g. NICE guidelines, national dementia and carer strategies/ plans and all relevant local PCN policies).

Recognise limits of own competency and professional boundaries.

Make appropriate and timely referral to meet the needs of people living with dementia and carers when required

Take responsibility for personal and professional development.

Respect confidentiality and privacy of people living with dementia, their carers and staff at all times and comply with local PCN and UCL policy on confidentiality, GDPR and ethical requirements relating to information held manually or on computerised systems.

  • - Health care professional with specialist expertise in dementia care and evidence of continuing professional development, such as a Registered Nurse (Adult/Mental Health) or equivalent Degree in AHP area such as Occupational Therapy
  • - Experience of collaborative and multi-agency working
  • - Experience of delivering/co-facilitating education/ training
  • - First hand experience of the complexity of dementia case management, including: knowledge of dementia and how this affects day-to-day life, relationships, family and support networks; dementia/carer assessment; delivering therapeutic interventions (e.g. post-diagnotic counselling, solution focussed/problem-solving approaches, Advanced care planning); supporting other practitioners in managing multi-morbidity in the context of dementia
  • - Leadership/project management experience
  • - IT literate
  • - Evidence of excellent communication and interpersonal skills
  • - Skills in engaging, collaborating with and (where appropriate) influencing a range of stakeholders and multidisciplinary team working
  • - Excellent presentation and teaching skills
  • - Understanding of ethical and legal issues relating to dementia care (e.g., Knowledge of the Mental Health Act 1983, and the Mental Capacity Act & Deprivation of Liberty Safeguards)
